51b8c1fe25 vfs: keep inodes with page cache off the inode shrinker LRU
Historically (pre-2.5), the inode shrinker used to reclaim only empty
inodes and skip over those that still contained page cache.  This caused
problems on highmem hosts: struct inode could put fill lowmem zones
before the cache was getting reclaimed in the highmem zones.

To address this, the inode shrinker started to strip page cache to
facilitate reclaiming lowmem.  However, this comes with its own set of
problems: the shrinkers may drop actively used page cache just because
the inodes are not currently open or dirty - think working with a large
git tree.  It further doesn't respect cgroup memory protection settings
and can cause priority inversions between containers.

Nowadays, the page cache also holds non-resident info for evicted cache
pages in order to detect refaults.  We've come to rely heavily on this
data inside reclaim for protecting the cache workingset and driving swap
behavior.  We also use it to quantify and report workload health through
psi.  The latter in turn is used for fleet health monitoring, as well as
driving automated memory sizing of workloads and containers, proactive
reclaim and memory offloading schemes.

The consequences of dropping page cache prematurely is that we're seeing
subtle and not-so-subtle failures in all of the above-mentioned
scenarios, with the workload generally entering unexpected thrashing
states while losing the ability to reliably detect it.

To fix this on non-highmem systems at least, going back to rotating
inodes on the LRU isn't feasible.  We've tried (commit a76cf1a474
("mm: don't reclaim inodes with many attached pages")) and failed
(commit 69056ee6a8 ("Revert "mm: don't reclaim inodes with many
attached pages"")).

The issue is mostly that shrinker pools attract pressure based on their
size, and when objects get skipped the shrinkers remember this as
deferred reclaim work.  This accumulates excessive pressure on the
remaining inodes, and we can quickly eat into heavily used ones, or
dirty ones that require IO to reclaim, when there potentially is plenty
of cold, clean cache around still.

Instead, this patch keeps populated inodes off the inode LRU in the
first place - just like an open file or dirty state would.  An otherwise
clean and unused inode then gets queued when the last cache entry
disappears.  This solves the problem without reintroducing the reclaim
issues, and generally is a bit more scalable than having to wade through
potentially hundreds of thousands of busy inodes.

Locking is a bit tricky because the locks protecting the inode state
(i_lock) and the inode LRU (lru_list.lock) don't nest inside the
irq-safe page cache lock (i_pages.xa_lock).  Page cache deletions are
serialized through i_lock, taken before the i_pages lock, to make sure
depopulated inodes are queued reliably.  Additions may race with
deletions, but we'll check again in the shrinker.  If additions race
with the shrinker itself, we're protected by the i_lock: if find_inode()
or iput() win, the shrinker will bail on the elevated i_count or
I_REFERENCED; if the shrinker wins and goes ahead with the inode, it
will set I_FREEING and inhibit further igets(), which will cause the
other side to create a new instance of the inode instead.

7ae12c809f fs: inode: count invalidated shadow pages in pginodesteal
pginodesteal is supposed to capture the impact that inode reclaim has on
the page cache state.  Currently, it doesn't consider shadow pages that
get dropped this way, even though this can have a significant impact on
paging behavior, memory pressure calculations etc.

To improve visibility into these effects, make sure shadow pages get
counted when they get dropped through inode reclaim.

This changes the return value semantics of invalidate_mapping_pages()
semantics slightly, but the only two users are the inode shrinker itsel
and a usb driver that logs it for debugging purposes.

The page cache deletion paths all have interrupts enabled, so no need to
use irqsafe/irqrestore locking variants.

They used to have irqs disabled by the memcg lock added in commit
c4843a7593 ("memcg: add per cgroup dirty page accounting"), but that has
since been replaced by memcg taking the page lock instead, commit
0a31bc97c8 ("mm: memcontrol: rewrite uncharge AP").

730633f0b7 mm: Protect operations adding pages to page cache with invalidate_lock
Currently, serializing operations such as page fault, read, or readahead
against hole punching is rather difficult. The basic race scheme is
like:

fallocate(FALLOC_FL_PUNCH_HOLE)			read / fault / ..
  truncate_inode_pages_range()
						  <create pages in page
						   cache here>
  <update fs block mapping and free blocks>

Now the problem is in this way read / page fault / readahead can
instantiate pages in page cache with potentially stale data (if blocks
get quickly reused). Avoiding this race is not simple - page locks do
not work because we want to make sure there are *no* pages in given
range. inode->i_rwsem does not work because page fault happens under
mmap_sem which ranks below inode->i_rwsem. Also using it for reads makes
the performance for mixed read-write workloads suffer.

So create a new rw_semaphore in the address_space - invalidate_lock -
that protects adding of pages to page cache for page faults / reads /
readahead.

22061a1ffa mm/thp: unmap_mapping_page() to fix THP truncate_cleanup_page()
There is a race between THP unmapping and truncation, when truncate sees
pmd_none() and skips the entry, after munmap's zap_huge_pmd() cleared
it, but before its page_remove_rmap() gets to decrement
compound_mapcount: generating false "BUG: Bad page cache" reports that
the page is still mapped when deleted.  This commit fixes that, but not
in the way I hoped.

The first attempt used try_to_unmap(page, TTU_SYNC|TTU_IGNORE_MLOCK)
instead of unmap_mapping_range() in truncate_cleanup_page(): it has
often been an annoyance that we usually call unmap_mapping_range() with
no pages locked, but there apply it to a single locked page.
try_to_unmap() looks more suitable for a single locked page.

However, try_to_unmap_one() contains a VM_BUG_ON_PAGE(!pvmw.pte,page):
it is used to insert THP migration entries, but not used to unmap THPs.
Copy zap_huge_pmd() and add THP handling now? Perhaps, but their TLB
needs are different, I'm too ignorant of the DAX cases, and couldn't
decide how far to go for anon+swap.  Set that aside.

The second attempt took a different tack: make no change in truncate.c,
but modify zap_huge_pmd() to insert an invalidated huge pmd instead of
clearing it initially, then pmd_clear() between page_remove_rmap() and
unlocking at the end.  Nice.  But powerpc blows that approach out of the
water, with its serialize_against_pte_lookup(), and interesting pgtable
usage.  It would need serious help to get working on powerpc (with a
minor optimization issue on s390 too).  Set that aside.

Just add an "if (page_mapped(page)) synchronize_rcu();" or other such
delay, after unmapping in truncate_cleanup_page()? Perhaps, but though
that's likely to reduce or eliminate the number of incidents, it would
give less assurance of whether we had identified the problem correctly.

This successful iteration introduces "unmap_mapping_page(page)" instead
of try_to_unmap(), and goes the usual unmap_mapping_range_tree() route,
with an addition to details.  Then zap_pmd_range() watches for this
case, and does spin_unlock(pmd_lock) if so - just like
page_vma_mapped_walk() now does in the PVMW_SYNC case.  Not pretty, but
safe.

Note that unmap_mapping_page() is doing a VM_BUG_ON(!PageLocked) to
assert its interface; but currently that's only used to make sure that
page->mapping is stable, and zap_pmd_range() doesn't care if the page is
locked or not.  Along these lines, in invalidate_inode_pages2_range()
move the initial unmap_mapping_range() out from under page lock, before
then calling unmap_mapping_page() under page lock if still mapped.

7716506ada mm: introduce and use mapping_empty()
Patch series "Remove nrexceptional tracking", v2.

We actually use nrexceptional for very little these days.  It's a minor
pain to keep in sync with nrpages, but the pain becomes much bigger with
the THP patches because we don't know how many indices a shadow entry
occupies.  It's easier to just remove it than keep it accurate.

Also, we save 8 bytes per inode which is nothing to sneeze at; on my
laptop, it would improve shmem_inode_cache from 22 to 23 objects per
16kB, and inode_cache from 26 to 27 objects.  Combined, that saves
a megabyte of memory from a combined usage of 25MB for both caches.
Unfortunately, ext4 doesn't cross a magic boundary, so it doesn't save
any memory for ext4.

This patch (of 4):

Instead of checking the two counters (nrpages and nrexceptional), we can
just check whether i_pages is empty.

5c211ba29d mm: add and use find_lock_entries
We have three functions (shmem_undo_range(), truncate_inode_pages_range()
and invalidate_mapping_pages()) which want exactly this function, so add
it to filemap.c.  Before this patch, shmem_undo_range() would split any
compound page which overlaps either end of the range being punched in both
the first and second loops through the address space.  After this patch,
that functionality is left for the second loop, which is arguably more
appropriate since the first loop is supposed to run through all the pages
quickly, and splitting a page can sleep.

a00cda3f0a mm: fix kernel-doc markups
Kernel-doc markups should use this format:
        identifier - description

Fix some issues on mm files:

1) The definition for get_user_pages_locked() doesn't follow it.  Also,
   it expects a short descrpition at the header, followed by a long one,
   after the parameters.  Fix it.

2) Kernel-doc requires that a kernel-doc markup to be immediately below
   the function prototype, as otherwise it will rename it.  So, move
   get_pfnblock_flags_mask() description to the right place.

3) Make invalidate_mapping_pagevec() to also follow the expected
   kernel-doc format.

While here, fix a few minor English syntax issues, as suggested
by Matthew:
	will used -> will be used
	similar with -> similar to

eb1d7a65f0 mm, fadvise: improve the expensive remote LRU cache draining after FADV_DONTNEED
Our users reported that there're some random latency spikes when their RT
process is running.  Finally we found that latency spike is caused by
FADV_DONTNEED.  Which may call lru_add_drain_all() to drain LRU cache on
remote CPUs, and then waits the per-cpu work to complete.  The wait time
is uncertain, which may be tens millisecond.

That behavior is unreasonable, because this process is bound to a specific
CPU and the file is only accessed by itself, IOW, there should be no
pagecache pages on a per-cpu pagevec of a remote CPU.  That unreasonable
behavior is partially caused by the wrong comparation of the number of
invalidated pages and the number of the target.  For example,

        if (count < (end_index - start_index + 1))

The count above is how many pages were invalidated in the local CPU, and
(end_index - start_index + 1) is how many pages should be invalidated.
The usage of (end_index - start_index + 1) is incorrect, because they are
virtual addresses, which may not mapped to pages.  Besides that, there may
be holes between start and end.  So we'd better check whether there are
still pages on per-cpu pagevec after drain the local cpu, and then decide
whether or not to call lru_add_drain_all().

After I applied it with a hotfix to our production environment, most of
the lru_add_drain_all() can be avoided.

If all pages are deleted from the mapping by memory reclaim and also
moved to the cleancache:

__delete_from_page_cache
  (no shadow case)
  unaccount_page_cache_page
    cleancache_put_page
  page_cache_delete
    mapping->nrpages -= nr
    (nrpages becomes 0)

We don't clean the cleancache for an inode after final file truncation
(removal).

truncate_inode_pages_final
  check (nrpages || nrexceptional) is false
    no truncate_inode_pages
      no cleancache_invalidate_inode(mapping)

These way when reading the new file created with same inode we may get
these trash leftover pages from cleancache and see wrong data instead of
the contents of the new file.

Fix it by always doing truncate_inode_pages which is already ready for
nrpages == 0 && nrexceptional == 0 case and just invalidates inode.

3159f943aa xarray: Replace exceptional entries
Introduce xarray value entries and tagged pointers to replace radix
tree exceptional entries.  This is a slight change in encoding to allow
the use of an extra bit (we can now store BITS_PER_LONG - 1 bits in a
value entry).  It is also a change in emphasis; exceptional entries are
intimidating and different.  As the comment explains, you can choose
to store values or pointers in the xarray and they are both first-class
citizens.

977fbdcd59 mm: add unmap_mapping_pages()
Several users of unmap_mapping_range() would prefer to express their
range in pages rather than bytes.  Unfortuately, on a 32-bit kernel, you
have to remember to cast your page number to a 64-bit type before
shifting it, and four places in the current tree didn't remember to do
that.  That's a sign of a bad interface.

Conveniently, unmap_mapping_range() actually converts from bytes into
pages, so hoist the guts of unmap_mapping_range() into a new function
unmap_mapping_pages() and convert the callers which want to use pages.

8667982014 mm, pagevec: remove cold parameter for pagevecs
Every pagevec_init user claims the pages being released are hot even in
cases where it is unlikely the pages are hot.  As no one cares about the
hotness of pages being released to the allocator, just ditch the
parameter.

No performance impact is expected as the overhead is marginal.  The
parameter is removed simply because it is a bit stupid to have a useless
parameter copied everywhere.

f218759918 mm, truncate: remove all exceptional entries from pagevec under one lock
During truncate each entry in a pagevec is checked to see if it is an
exceptional entry and if so, the shadow entry is cleaned up.  This is
potentially expensive as multiple entries for a mapping locks/unlocks
the tree lock.  This batches the operation such that any exceptional
entries removed from a pagevec only acquire the mapping tree lock once.
The corner case where this is more expensive is where there is only one
exceptional entry but this is unlikely due to temporal locality and how
it affects LRU ordering.  Note that for truncations of small files
created recently, this patch should show no gain because it only batches
the handling of exceptional entries.

During truncation, the mapping has already been checked for shmem and
dax so it's known that workingset_update_node is required.

This patch avoids the checks on mapping for each page being truncated.
In all other cases, a lookup helper is used to determine if
workingset_update_node() needs to be called.  The one danger is that the
API is slightly harder to use as calling workingset_update_node directly
without checking for dax or shmem mappings could lead to surprises.
However, the API rarely needs to be used and hopefully the comment is
enough to give people the hint.

As you'd expect, the gain is marginal but it can be detected.  The
differences in bonnie are all within the noise which is not surprising
given the impact on the microbenchmark.

radix_tree_update_node_t is a callback for some radix operations that
optionally passes in a private field.  The only user of the callback is
workingset_update_node and as it no longer requires a mapping, the
private field is removed.

aa65c29ce1 mm: batch radix tree operations when truncating pages
Currently we remove pages from the radix tree one by one.  To speed up
page cache truncation, lock several pages at once and free them in one
go.  This allows us to batch radix tree operations in a more efficient
way and also save round-trips on mapping->tree_lock.  As a result we
gain about 20% speed improvement in page cache truncation.

Data from a simple benchmark timing 10000 truncates of 1024 pages (on
ext4 on ramdisk but the filesystem is barely visible in the profiles).
The range shows 1% and 95% percentiles of the measured times:

76b6f9b7ed mm/truncate.c: fix THP handling in invalidate_mapping_pages()
The condition checking for THP straddling end of invalidated range is
wrong - it checks 'index' against 'end' but 'index' has been already
advanced to point to the end of THP and thus the condition can never be
true.  As a result THP straddling 'end' has been fully invalidated.
Given the nature of invalidate_mapping_pages(), this could be only
performance issue.  In fact, we are lucky the condition is wrong because
if it was ever true, we'd leave locked page behind.

Fix the condition checking for THP straddling 'end' and also properly
unlock the page.  Also update the comment before the condition to
explain why we decide not to invalidate the page as it was not clear to
me and I had to ask Kirill.

cd656375f9 mm: fix data corruption due to stale mmap reads
Currently, we didn't invalidate page tables during invalidate_inode_pages2()
for DAX.  That could result in e.g. 2MiB zero page being mapped into
page tables while there were already underlying blocks allocated and
thus data seen through mmap were different from data seen by read(2).
The following sequence reproduces the problem:

 - open an mmap over a 2MiB hole

 - read from a 2MiB hole, faulting in a 2MiB zero page

 - write to the hole with write(3p). The write succeeds but we
   incorrectly leave the 2MiB zero page mapping intact.

 - via the mmap, read the data that was just written. Since the zero
   page mapping is still intact we read back zeroes instead of the new
   data.

Fix the problem by unconditionally calling invalidate_inode_pages2_range()
in dax_iomap_actor() for new block allocations and by properly
invalidating page tables in invalidate_inode_pages2_range() for DAX
mappings.

Patch series "mm,dax: Fix data corruption due to mmap inconsistency",
v4.

This series fixes data corruption that can happen for DAX mounts when
page faults race with write(2) and as a result page tables get out of
sync with block mappings in the filesystem and thus data seen through
mmap is different from data seen through read(2).

The series passes testing with t_mmap_stale test program from Ross and
also other mmap related tests on DAX filesystem.

This patch (of 4):

dax_invalidate_mapping_entry() currently removes DAX exceptional entries
only if they are clean and unlocked.  This is done via:

  invalidate_mapping_pages()
    invalidate_exceptional_entry()
      dax_invalidate_mapping_entry()

However, for page cache pages removed in invalidate_mapping_pages()
there is an additional criteria which is that the page must not be
mapped.  This is noted in the comments above invalidate_mapping_pages()
and is checked in invalidate_inode_page().

For DAX entries this means that we can can end up in a situation where a
DAX exceptional entry, either a huge zero page or a regular DAX entry,
could end up mapped but without an associated radix tree entry.  This is
inconsistent with the rest of the DAX code and with what happens in the
page cache case.

We aren't able to unmap the DAX exceptional entry because according to
its comments invalidate_mapping_pages() isn't allowed to block, and
unmap_mapping_range() takes a write lock on the mapping->i_mmap_rwsem.

Since we essentially never have unmapped DAX entries to evict from the
radix tree, just remove dax_invalidate_mapping_entry().

3a4f8a0b3f mm: remove shmem_mapping() shmem_zero_setup() duplicates
Remove the prototypes for shmem_mapping() and shmem_zero_setup() from
linux/mm.h, since they are already provided in linux/shmem_fs.h.  But
shmem_fs.h must then provide the inline stub for shmem_mapping() when
CONFIG_SHMEM is not set, and a few more cfiles now need to #include it.

c6dcf52c23 mm: Invalidate DAX radix tree entries only if appropriate
Currently invalidate_inode_pages2_range() and invalidate_mapping_pages()
just delete all exceptional radix tree entries they find. For DAX this
is not desirable as we track cache dirtiness in these entries and when
they are evicted, we may not flush caches although it is necessary. This
can for example manifest when we write to the same block both via mmap
and via write(2) (to different offsets) and fsync(2) then does not
properly flush CPU caches when modification via write(2) was the last
one.

Create appropriate DAX functions to handle invalidation of DAX entries
for invalidate_inode_pages2_range() and invalidate_mapping_pages() and
wire them up into the corresponding mm functions.

14b468791f mm: workingset: move shadow entry tracking to radix tree exceptional tracking
Currently, we track the shadow entries in the page cache in the upper
bits of the radix_tree_node->count, behind the back of the radix tree
implementation.  Because the radix tree code has no awareness of them,
we rely on random subtleties throughout the implementation (such as the
node->count != 1 check in the shrinking code, which is meant to exclude
multi-entry nodes but also happens to skip nodes with only one shadow
entry, as that's accounted in the upper bits).  This is error prone and
has, in fact, caused the bug fixed in d3798ae8c6 ("mm: filemap: don't
plant shadow entries without radix tree node").

To remove these subtleties, this patch moves shadow entry tracking from
the upper bits of node->count to the existing counter for exceptional
entries.  node->count goes back to being a simple counter of valid
entries in the tree node and can be shrunk to a single byte.

This vastly simplifies the page cache code.  All accounting happens
natively inside the radix tree implementation, and maintaining the LRU
linkage of shadow nodes is consolidated into a single function in the
workingset code that is called for leaf nodes affected by a change in
the page cache tree.

This also removes the last user of the __radix_delete_node() return
value.  Eliminate it.

6d75f366b9 lib: radix-tree: check accounting of existing slot replacement users
The bug in khugepaged fixed earlier in this series shows that radix tree
slot replacement is fragile; and it will become more so when not only
NULL<->!NULL transitions need to be caught but transitions from and to
exceptional entries as well.  We need checks.

Re-implement radix_tree_replace_slot() on top of the sanity-checked
__radix_tree_replace().  This requires existing callers to also pass the
radix tree root, but it'll warn us when somebody replaces slots with
contents that need proper accounting (transitions between NULL entries,
real entries, exceptional entries) and where a replacement through the
slot pointer would corrupt the radix tree node counts.

5cbc198ae0 mm: fix false-positive WARN_ON() in truncate/invalidate for hugetlb
Hugetlb pages have ->index in size of the huge pages (PMD_SIZE or
PUD_SIZE), not in PAGE_SIZE as other types of pages.  This means we
cannot user page_to_pgoff() to check whether we've got the right page
for the radix-tree index.

Let's introduce page_to_index() which would return radix-tree index for
given page.

We will be able to get rid of this once hugetlb will be switched to
multi-order entries.

And we are in a situation where we fail in dax_radix_entry() with -EIO.

Another problem with the current DAX page fault locking is that there is
no race-free way to clear dirty tag in the radix tree. We can always
end up with clean radix tree and dirty data in CPU cache.

We fix the first problem by introducing locking of exceptional radix
tree entries in DAX mappings acting very similarly to page lock and thus
synchronizing properly faults against the same mapping index. The same
lock can later be used to avoid races when clearing radix tree dirty
tag.

09cbfeaf1a mm, fs: get rid of PAGE_CACHE_* and page_cache_{get,release} macros
PAGE_CACHE_{SIZE,SHIFT,MASK,ALIGN} macros were introduced *long* time
ago with promise that one day it will be possible to implement page
cache with bigger chunks than PAGE_SIZE.

This promise never materialized.  And unlikely will.

We have many places where PAGE_CACHE_SIZE assumed to be equal to
PAGE_SIZE.  And it's constant source of confusion on whether
PAGE_CACHE_* or PAGE_* constant should be used in a particular case,
especially on the border between fs and mm.

Global switching to PAGE_CACHE_SIZE != PAGE_SIZE would cause to much
breakage to be doable.

Let's stop pretending that pages in page cache are special.  They are
not.

The changes are pretty straight-forward:

 - <foo> << (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

 - <foo> >> (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

This patch contains automated changes generated with coccinelle using
script below.  For some reason, coccinelle doesn't patch header files.
I've called spatch for them manually.

The only adjustment after coccinelle is revert of changes to
PAGE_CAHCE_ALIGN definition: we are going to drop it later.

There are few places in the code where coccinelle didn't reach.  I'll
fix them manually in a separate patch.  Comments and documentation also
will be addressed with the separate patch.

fdf1cdb91b mm: remove unnecessary uses of lock_page_memcg()
There are several users that nest lock_page_memcg() inside lock_page()
to prevent page->mem_cgroup from changing.  But the page lock prevents
pages from moving between cgroups, so that is unnecessary overhead.

Remove lock_page_memcg() in contexts with locked contexts and fix the
debug code in the page stat functions to be okay with the page lock.

81f8c3a461 mm: memcontrol: generalize locking for the page->mem_cgroup binding
These patches tag the page cache radix tree eviction entries with the
memcg an evicted page belonged to, thus making per-cgroup LRU reclaim
work properly and be as adaptive to new cache workingsets as global
reclaim already is.

This should have been part of the original thrash detection patch
series, but was deferred due to the complexity of those patches.

This patch (of 5):

So far the only sites that needed to exclude charge migration to
stabilize page->mem_cgroup have been per-cgroup page statistics, hence
the name mem_cgroup_begin_page_stat().  But per-cgroup thrash detection
will add another site that needs to ensure page->mem_cgroup lifetime.

Rename these locking functions to the more generic lock_page_memcg() and
unlock_page_memcg().  Since charge migration is a cgroup1 feature only,
we might be able to delete it at some point, and these now easy to
identify locking sites along with it.

f9fe48bece dax: support dirty DAX entries in radix tree
Add support for tracking dirty DAX entries in the struct address_space
radix tree.  This tree is already used for dirty page writeback, and it
already supports the use of exceptional (non struct page*) entries.

In order to properly track dirty DAX pages we will insert new
exceptional entries into the radix tree that represent dirty DAX PTE or
PMD pages.  These exceptional entries will also contain the writeback
addresses for the PTE or PMD faults that we can use at fsync/msync time.

There are currently two types of exceptional entries (shmem and shadow)
that can be placed into the radix tree, and this adds a third.  We rely
on the fact that only one type of exceptional entry can be found in a
given radix tree based on its usage.  This happens for free with DAX vs
shmem but we explicitly prevent shadow entries from being added to radix
trees for DAX mappings.

The only shadow entries that would be generated for DAX radix trees
would be to track zero page mappings that were created for holes.  These
pages would receive minimal benefit from having shadow entries, and the
choice to have only one type of exceptional entry in a given radix tree
makes the logic simpler both in clear_exceptional_entry() and in the
rest of DAX.
