c3c0fa9d94 vfio: clean up the check for mediated device in vfio_iommu_type1 
Pass the group flags to ->attach_group and remove the messy check for
the bus type.

8cc02d22d7 vfio: move the vfio_iommu_driver_ops interface out of <linux/vfio.h> 
Create a new private drivers/vfio/vfio.h header for the interface between
the VFIO core and the iommu drivers.
