2d20c72c02 setup correct int pipe type in ar9170_usb_exec_cmd
An int urb is constructed but we fill it in with a bulk pipe type.

Commit f661c6f8c6 implemented a pipe type
check when CONFIG_USB_DEBUG is enabled. The check failed for all the ar9170
usb transfers and the driver could not configure the wifi dongle.

This went unnoticed until now because most people don't have
CONFIG_USB_DEBUG enabled.

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

984b3f5746 bitops: rename for_each_bit() to for_each_set_bit()
Rename for_each_bit to for_each_set_bit in the kernel source tree.  To
permit for_each_clear_bit(), should that ever be added.

The patch includes a macro to map the old for_each_bit() onto the new
for_each_set_bit().  This is a (very) temporary thing to ease the migration.

535765179f ar9170: load firmware asynchronously
This converts ar9170 to load firmware asynchronously
out of ->probe() and only register with mac80211 when
all firmware has been loaded successfully. If, on the
other hand, any firmware fails to load, it will now
unbind from the device.

d1ace8e57a ar9170: remove get_tx_stats() mac80211 op
get_tx_stats() will be removed from mac80211.

Because ar9170 uses ieee80211_tx_queue_stats internally, create a new
identical struct ar9170_tx_queue_stats which the driver can use.

1ed32e4fc8 mac80211: remove struct ieee80211_if_init_conf

15295380f4 ar9170: Add support for D-Link DWA 160 A2
At least two revisions of the D-Link DWA 160 exist, called A1 and A2. A1
(USB-ID 07d1:3c10) is already listed in usb.c as D-Link DWA 160A. A2
(USB-ID 07d1:3a09) works if added to ar9170_usb_ids. I didn't do much
testing until now, but I was able to connect to APs using WPA or WEP and
transmit data.

Summary:

* Add model revision number to the comment for D-Link DWA 160 A1 (07d1:3c10)
* Add support for D-Link DWA 160 A2 (07d1:3a09)

c951ad3550 mac80211: convert aggregation to operate on vifs/stas
The entire aggregation code currently operates on the
hw pointer and station addresses, but that needs to
change to make stations purely per-vif; As one step
preparing for that make the aggregation code callable
with the station, or by the combination of virtual
interface and station address.

The current A-MPDU tx_status report mechanism is too inaccurate.

bcd8f54a84 atheros: use get_unaligned_le*() for bssid mask setting
Historically some macro helpers have been users for this,
AR5K_LOW_ID() and AR5K_HIGH_ID(), use upstream unaligned
helpers instead. This applid to ath5k and ar9170. ath9k
already uses this.

Worth noting is ath5k uses an ah_sta_id but that is already
the MAC address combined with the associaiton ID, ah_sta_id
is really ETH_ALEN in size.

e16c1bb67a ar9170: fix bug in iq-auto calibration value calculation
This patch fixes a embarrassing bug which was introduced by:
"[PATCH] ar9170: implement frequency calibration for one-stage/openfw"

The phy_data variable initialization has to done outside the for-loop
scope. This is because the for-loop uses u32 phy_data variable more
like a 4-byte field. But in each run only a single byte is calculated.
Therefore phy_data content needs to stay the same for at least 3 more
iterations, before the complete set can be uploaded.

fea6734a0e ath,ar9170: implemented conformance test limit calc. for tx power
apply the conformance test limits (CTL) stored in the eeprom upon
the values calculated for the tx power (ar->power_*).

This is based on the implementation in the vendor driver
(hal/hpmain.c, line 3700 ff.) with one difference:
If any ctl mode isn't found in the eeprom, we fall back to the "lower",
legacy modes (5GHT20,11A or 2GHT20,11G,11B). Otus only did 5GHT20->11A.

Currently CTL are applied for the FCC group only.

d9c35a506e ar9170: implement frequency calibration for one-stage/openfw
This patch ports some code from the vendor driver, which is
supposed to upload the right calibration values for the
chosen frequency.

In theory, this should give a better range and throughput
for all users with the open, or one-stage firmware.

3ac64beecd mac80211: allow configure_filter callback to sleep
Over time, a whole bunch of drivers have come up
with their own scheme to delay the configure_filter
operation to a workqueue. To be able to simplify
things, allow configure_filter to sleep, and add
a new prepare_multicast callback that drivers that
need the multicast address list implement. This new
callback must be atomic, but most drivers either
don't care or just calculate a hash which can be
done atomically and then uploaded to the hardware
non-atomically.

A cursory look suggests that at76c50x-usb, ar9170,
mwl8k (which is actually very broken now), rt2x00,
wl1251, wl1271 and zd1211 should make use of this
new capability.

42935ecaf4 mac80211: redefine usage of the mac80211 workqueue
The mac80211 workqueue exists to enable mac80211 and drivers
to queue their own work on a single threaded workqueue. mac80211
takes care to flush the workqueue during suspend but we never
really had requirements on drivers for how they should use
the workqueue in consideration for suspend.

We extend mac80211 to document how the mac80211 workqueue should
be used, how it should not be used and finally move raw access to
the workqueue to mac80211 only. Drivers and mac80211 use helpers
to queue work onto the mac80211 workqueue:

  * ieee80211_queue_work()
  * ieee80211_queue_delayed_work()

These helpers will now warn if mac80211 already completed its
suspend cycle and someone is trying to queue work. mac80211
flushes the mac80211 workqueue prior to suspend a few times,
but we haven't taken the care to ensure drivers won't add more
work after suspend. To help with this we add a warning when
someone tries to add work and mac80211 already completed the
suspend cycle.

Drivers should ensure they cancel any work or delayed work
in the mac80211 stop() callback.

e351cfbf4e ar9170: remove unneeded flush_workqueue()
cancel_delayed_work_sync() and cancel_work_sync() are already being
used therefore already waiting for all pending work by the driver
to have been completed, no need to flush the mac80211 workqueue.
