51e3b2ad6d scsi: mpt3sas: Load balance to improve performance and avoid soft lockups
Driver uses "reply descriptor post queues" in round robin fashion so that
IO's are distributed to all the available reply descriptor post queues
equally.  With this each reply descriptor post queue load is balanced.

This is enabled only if CPUs count to MSI-X vector count ratio is X:1
(where X > 1) This improves performance and also fixes soft lockups.

320e77acb3 scsi: mpt3sas: Irq poll to avoid CPU hard lockups
Issue Description:
We have seen cpu lock up issue from fields if system has greater (more than
96) logical cpu count.  SAS3.0 controller (Invader series) supports at max
96 msix vector and SAS3.5 product (Ventura) supports at max 128 msix
vectors.

This may be a generic issue (if PCI device supports completion on multiple
reply queues).  Let me explain it w.r.t to mpt3sas supported h/w just to
simplify the problem and possible changes to handle such issues. IT HBA
(mpt3sas) supports multiple reply queues in completion path. Driver creates
MSI-x vectors for controller as "min of (FW supported Reply queue, Logical
CPUs)". If submitter is not interrupted via completion on same CPU, there
is a loop in the IO path. This behavior can cause hard/soft CPU lockups, IO
timeout, system sluggish etc.

Example - one CPU (e.g. CPU A) is busy submitting the IOs and another CPU
(e.g. CPU B) is busy with processing the corresponding IO's reply
descriptors from reply descriptor queue upon receiving the interrupts from
HBA.  If the CPU A is continuously pumping the IOs then always CPU B (which
is executing the ISR) will see the valid reply descriptors in the reply
descriptor queue and it will be continuously processing those reply
descriptor in a loop without quitting the ISR handler.

Mpt3sas driver will exit ISR handler if it finds unused reply descriptor in
the reply descriptor queue. Since CPU A will be continuously sending the
IOs, CPU B may always see a valid reply descriptor (posted by HBA Firmware
after processing the IO) in the reply descriptor queue. In worst case,
driver will not quit from this loop in the ISR handler. Eventually, CPU
lockup will be detected by watchdog.

Above mentioned behavior is not common if "rq_affinity" set to 2 or
affinity_hint is honored by irqbalance as "exact". If rq_affinity is set
to 2, submitter will be always interrupted via completion on same CPU.  If
irqbalance is using "exact" policy, interrupt will be delivered to
submitter CPU.

If CPU counts to MSI-X vectors (reply descriptor Queues) count ratio is not
1:1, we still have exposure of issue explained above and for that we don't
have any solution.

Exposure of soft/hard lockup if CPU count is more than MSI-x supported by
device.

If CPUs count to MSI-x vectors count ratio is not 1:1, (Other way, if CPU
counts to MSI-x vector count ratio is something like X:1, where X > 1) then
'exact' irqbalance policy OR rq_affinity = 2 won't help to avoid CPU
hard/soft lockups. There won't be any one to one mapping between CPU to
MSI-x vector instead one MSI-x interrupt (or reply descriptor queue) is
shared with group/set of CPUs and there is a possibility of having a loop
in the IO path within that CPU group and may observe lockups.

For example: Consider a system having two NUMA nodes and each node having
four logical CPUs and also consider that number of MSI-x vectors enabled on
the HBA is two, then CPUs count to MSI-x vector count ratio as 4:1.  e.g.
MSIx vector 0 is affinity to CPU 0, CPU 1, CPU 2 & CPU 3 of NUMA node 0 and
MSI-x vector 1 is affinity to CPU 4, CPU 5, CPU 6 & CPU 7 of NUMA node 1.

Assume that user started an application which uses all the CPUs of NUMA
node 0 for issuing the IOs.  Only one CPU from affinity list (it can be any
cpu since this behavior depends upon irqbalance) CPU0 will receive the
interrupts from MSIx vector 0 for all the IOs. Eventually, CPU 0 IO
submission percentage will be decreasing and ISR processing percentage will
be increasing as it is more busy with processing the interrupts.  Gradually
IO submission percentage on CPU 0 will be zero and it's ISR processing
percentage will be 100 percentage as IO loop has already formed within the
NUMA node 0, i.e. CPU 1, CPU 2 & CPU 3 will be continuously busy with
submitting the heavy IOs and only CPU 0 is busy in the ISR path as it
always find the valid reply descriptor in the reply descriptor
queue. Eventually, we will observe the hard lockup here.

Chances of occurring of hard/soft lockups are directly proportional to
value of X. If value of X is high, then chances of observing CPU lockups is
high.

Solution: Use IRQ poll interface defined in " irq_poll.c".  mpt3sas driver
will execute ISR routine in Softirq context and it will always quit the
loop based on budget provided in IRQ poll interface.

In these scenarios (i.e. where CPUs count to MSI-X vectors count ratio is
X:1 (where X > 1)), IRQ poll interface will avoid CPU hard lockups due to
voluntary exit from the reply queue processing based on budget.  Note -
Only one MSI-x vector is busy doing processing.

23c3828aa2 scsi: mpt3sas: fix memory ordering on 64bit writes
With commit 09c2f95ad4 ("scsi: mpt3sas: Swap I/O memory read value back
to cpu endianness"), 64bit writes in _base_writeq() were rewritten to use
__raw_writeq() instad of writeq().

This introduced a bug apparent on powerpc64 systems such as the Raptor
Talos II that causes the HBA to drop from the PCIe bus under heavy load and
being reinitialized after a couple of seconds.

It can easily be triggered on affacted systems by using something like

  fio --name=random-write --iodepth=4 --rw=randwrite --bs=4k --direct=0 \
    --size=128M --numjobs=64 --end_fsync=1
  fio --name=random-write --iodepth=4 --rw=randwrite --bs=64k --direct=0 \
    --size=128M --numjobs=64 --end_fsync=1

a couple of times. In my case I tested it on both a ZFS raidz2 and a btrfs
raid6 using LSI 9300-8i and 9400-8i controllers.

The fix consists in resembling the write ordering of writeq() by adding a
mandatory write memory barrier before device access and a compiler barrier
afterwards. The additional MMIO barrier is superfluous.

b899202901 scsi: mpt3sas: Add separate function for aero doorbell reads
Sometimes Aero controllers appears to be returning bad data (0) for
doorbell register read and if retries are performed immediately after the
bad read, they return good data.

Workaround is added to retry read from doorbell registers for maximum three
times if driver get the zero.  Added functions base_readl_aero for Aero IOC
and base_readl for gen35 and other controllers.

6cd1bc7b9b scsi: mpt3sas: Don't modify EEDPTagMode field setting on SAS3.5 HBA devices
If EEDPTagMode field in manufacturing page11 is set then unset it. This is
needed to fix a hardware bug only in SAS3/SAS2 cards. So, skipping
EEDPTagMode changes in Manufacturing page11 for SAS 3.5 controllers.

 continuing absence of Nic we have target updates for tcmu and target
 core (all with reviews and acks).  The biggest observable change is
 going to be that we're (again) trying to switch to mulitqueue as the
 default (a user can still override the setting on the kernel command
 line).  Other major core stuff is the removal of the remaining
 Microchannel drivers, an update of the internal timers and some
 reworks of completion and result handling.

As a part of host reset operation, driver will flushout all IOs outstanding
at driver level with "DID_RESET" result.  To find which are all commands
outstanding at the driver level, driver loops with smid starting from one
to HBA queue depth and calls mpt3sas_scsih_scsi_lookup_get() to get scmd as
shown below

But in mpt3sas_scsih_scsi_lookup_get() function, driver returns some scsi
cmnds which are not outstanding at the driver level (possibly request is
constructed at block layer since QUEUE_FLAG_QUIESCED is not set. Even if
driver uses scsi_block_requests and scsi_unblock_requests, issue still
persists as they will be just blocking further IO from scsi layer and not
from block layer) and these commands are flushed with DID_RESET host bytes
thus resulting into above kernel BUG.

This issue got introduced by commit dbec4c9040 ("scsi: mpt3sas: lockless
command submission").

To fix this issue, we have modified the mpt3sas_scsih_scsi_lookup_get() to
check for smid equals to zero (note: whenever any scsi cmnd is processing
at the driver level then smid for that scsi cmnd will be non-zero, always
it starts from one) before it returns the scmd pointer to the caller. If
smid is zero then this function returns scmd pointer as NULL and driver
won't flushout those scsi cmnds at driver level with DID_RESET host byte
thus this issue will not be observed.

09c2f95ad4 scsi: mpt3sas: Swap I/O memory read value back to cpu endianness
Swap the I/O memory read value back to cpu endianness before storing it in
a data structures which are defined in the MPI headers where u8 components
are not defined in the endianness order.

In this area from day one mpt3sas driver is using le32_to_cpu() &
cpu_to_le32() APIs. But in commit cf6bf9710c
(mpt3sas: Bug fix for big endian systems) we have removed these APIs
before reading I/O memory which we should haven't done it. So
in this patch I am correcting it by adding these APIs back
before accessing I/O memory.

b15c9fd8bb scsi: mpt3sas: Fix for regression caused by sparse cleanups
Commit cf6bf9710c ("scsi: mpt3sas: Bug fix for big endian systems") was
merged to address sparse warnings. However, the patch introduced a
regression on big endian since the code accidentally mixed I/O memory
accessors, which do endian swaps, and regular CPU loads and stores.

Do a partial revert of the offending commit.

Since ioc->shost_recovery is set after ioc->reset_in_progress_mutex is
obtained, if concurrent resets are issued there is a short time during
which ioc->reset_in_progress_mutex is locked and ioc->shost_recovery ==
0. Avoid that this can cause trouble by unconditionally locking
ioc->shost_recovery.

1. For SAS3 controllers, driver should use combined reply queue when HBA

2. For SAS3.5 controllers, driver should use combined reply queue when HBA
supports more than 16 MSI-x vectors.

Modified driver code to use combined reply queue for SAS3 controllers when
HBA supports > 8 MSI-x vectors and for SAS3.5 controllers when HBA supports
> 16 MSI-x vectors.

d37306ca0e scsi: mpt3sas: Fix, False timeout prints for ioctl and other internal commands during controller reset.
When an ioctl is sent to FW, and if there is a controller reset issued
before ioctl gets completed, then in controller reset path all the pending
ioctl commands are terminated from "mpt3sas_ctl_reset_handler" function.
This will wake up the waiting ioctl commands in ioctl path and print
timeouts which are actually not timeouts.

Introduced "mpt3sas_base_check_cmd_timeout" function to check and print
whether command got timed out (or) terminated due to Host reset.

c1a6c5ac42 scsi: mpt3sas: For NVME device, issue a protocol level reset
1) Manufacturing Page 11 contains parameters to control internal
   firmware behavior. Based on AddlFlags2 field FW/Driver behaviour can
   be changed, (flag tm_custom_handling is used for this)

a) For PCIe device, protocol level reset should be used if flag
   tm_custom_handling is 0.  Since Abort Task Set, LUN reset and Target
   reset will result in a protocol level reset. Drivers should issue
   only one type of this reset, if that fails then it should escalate to
   a controller reset (diag reset/OCR).

b) If the driver has control over the TM reset timeout value, then
   driver should use the value exposed in PCIe Device Page 2 for pcie
   device (field ControllerResetTO).

22a923c315 scsi: mpt3sas: Cache enclosure pages during enclosure add.
In function _scsih_add_device, for each device connected to an
enclosure, driver reads the enclosure page(To get details like enclosure
handle, enclosure logical ID, enclosure level etc.)

With this patch, instead of reading enclosure page everytime, driver
maintains a list for enclosure device(During enclosure add event,
enclosure device is added to the list and removed from the list on
delete events) and uses the enclosure page from the list.

For every IO, memory of PAGE size is allocated for handling NVMe native
PRPS. And in addition to that for every IO (chains need per IO * chain
buffer size, e.g. 38 * 128byte) amount of memory is allocated for chain
buffers.

However, at any point of time; the IO request can be for NVMe target
device (where PRP's page is used for framing PRP's) or can be for SCSI
target device (where chain buffers are used for framing chain
SGE's). This patch modifies the driver to reuse same pre-allocated PRP
page buffers as a chain buffer for IO's targeted for SCSI target
devices. No need to allocate separate buffers for chain SGE's buffers.

Suppose if the number of chain buffers need for IO doesn't fit in the
PRP Page size then driver maintain's separate buffers for those extra
chain buffers that exceeds the PRP page size. For example consider PRP
page size as 4K and chain buffer size as 128 bytes, then number of chain
buffers that can fit in PRP page is 4096/128 => 32. if the number of
chain buffer need per IO exceeds 32; for example consider number of
chains need per IO is 36 then for remaining 4 chain buffer's driver
allocates them individual.

Both dma_addr_t and phys_addr_t may be wider than a pointer, so we must
avoid the conversion to pointer types. This also helps readability.

A second problem is treating MMIO addresses from a 'struct resource'
as addresses that can be used for DMA on that device. In almost all
cases, those are the same, but on some of the more obscure architectures,
PCI memory address 0 is mapped into the CPU address space at a nonzero
offset. I don't have a good fix for that, so I'm adding a comment here,
plus a WARN_ON() that triggers whenever the phys_addr_t number is
outside of the low 32-bit address space and causes a straight overflow
when assigned to the 32-bit sgel->Address.

Sending I/O through 32 bit descriptors to Ventura series of controller
results in IO timeout on certain conditions. This error only occurs on
systems with high I/O activity.

Changes in this patch will prevent driver from using 32 bit descriptor
and use 64 bit Descriptors

For MPI Endpoint/Mcpu, driver should double buffer data buffer/SGLs.
This is normally copied from host to internal memory of IOC by DMA
engine of PCI device. Since the interface to DMA from host to mCPU is
not present for Mcpu/MPI Endpoint device, driver does double copy of
those buffers directly to the mCPU memory region via BAR0 region.

Introduced API to calculate and return BAR0 mapped host buffer's
physical and virtual address for the provided smid.

c666d3be99 scsi: mpt3sas: wait for and flush running commands on shutdown/unload
This patch finishes all outstanding SCSI IO commands (but not other commands,
e.g., task management) in the shutdown and unload paths.

It first waits for the commands to complete (this is done after setting
'ioc->remove_host = 1 ', which prevents new commands to be queued) then it
flushes commands that might still be running.

This avoids triggering error handling (e.g., abort command) for all commands
possibly completed by the adapter after interrupts disabled.

cpu_msix_table is allocated to store online cpus, but pci_irq_get_affinity
may return cpu_possible_mask which is then used to access cpu_msix_table.

Performance improvement using block layer tag.

Curent driver gets scsiio tracker and free smid from link list and array
based tracking managed by driver.  Accessing list in main io path is
performance pentaly because of protection using spinlock
"scsi_lookup_lock".

In this patch:

1. Driver removes all link list access from main io path and
   use scmd->request->tag to get free smid.

2. Instead of holding 'struct scsiio_tracker' in its own pool
   driver can embed it into the scsi command.

Driver provides cmd_size in scsi_host_template, so that struct
scsiio_tracker is preallocated by scsi mid layer for each scsi command.

* Added support for translating the SGLs associated with incoming
commands either to IEE SGL or NVMe PRPs for NVMe devices.

* The hardware translation of IEEE SGL to NVMe PRPs has limitations
and if a command cannot be translated by hardware then it will go to
firmware and the firmware needs to translate it. This will have a
performance impact. To avoid that, the driver proactively checks
whether the translation will be done in hardware or not. If not, then
driver translates.

apply the following settings when reset_devices is set:

Earlier Active Cable Exception event with reason code "Cable Degraded
(0x02))" was added only for Active Cable. Now this event is extended to
Passive cable too. Re-arranged display message accordingly.

Also added Cable Exception Event event for SAS3008 & SAS3108 HBAs
(i.e. MPI 2.5 spec supporting HBAs). Previously, this event was enabled
only for MPI 2.6 spec supporting HBA devices.

b1391a5bf8 scsi: mpt3sas: remove redundant wmb
Due to relaxed ordering requirements on multiple architectures, drivers
are required to use wmb/rmb/mb combinations when they need to guarantee
observability between the memory and the HW.

The mpt3sas driver is already using wmb() for this purpose.  However, it
issues a writel following wmb(). writel() function on arm/arm64
arhictectures have an embedded wmb() call inside.

This results in unnecessary performance loss and code duplication.

writel already guarantees ordering for both cpu and bus. we don't need
additional wmb()

6b4c335a0f scsi: mpt3sas: Fix Firmware fault state 0x2100 during heavy 4K RR FIO stress test.
Due existence of loop in the IO path our HBA will receive heavy IOs and
also as driver is not updating the Reply Post Host Index frequently, So
there will be a high chance that our Firmware unable to find any free
entry in the Reply Post Descriptor Queue (i.e. Queue overflow occurs)
and can observe 0x2100 firmware fault.  So to fix this, we have defined
a thresh hold value. After continuously processing this thresh hold
number of reply descriptors driver will update the Reply Descriptor Host
Index so that this thresh hold number of reply descriptors entries will
be freed and these entries will be available for firmware and we won't
observe this Firmware fault. We have defined this threshold value as
1/3rd of the hba queue depth.

No. of MSIX vectors supported = min (Total no. of CPU cores,
MSIX vectors supported by card)

when RDPQ is disabled "max_msix_vectors" module parameter which was
declared as global was set to '8' and hence if there are more than one card
in system among which if RDPQ disabled card is enumerated first then only 8
MSIX vectors was getting enabled for all the cards(including RDPQ enabled
card,which can support more than 8 MSIX vectors).

Used local variable instead of global variable ,if RDPQ is disabled this
local variable is set to '8' else it is set to "max_msix_vectors" (by
default this is set to -1, whose value can be set by user during driver
load time).So now regardless of whether RDPQ disabled card is enumerated
first or RDPQ enabled card is enumerated first , MSIX vectors enabled
depends on the cards capability.

8bbb1cf63f mpt3sas: Fix warnings exposed by W=1
Trivial non-functional changes for a couple annoying things:

  1) Functions local to files are not declared static, which is
  frustrating when reading the code because it's non-obvious at first
  glance what's actually called from other files.

  2) Set-but-unused variables abound, presumably to mask -Wunused-result
  errors in the past. None of these are flagged today though (with one
  exception noted below), so remove them.

Fixing (2) exposed the fact that we improperly ignore the return value
of scsi_device_reprobe() in _scsih_reprobe_lun(). Fixing the calling
code to deal with the potential error is non-trivial, so for now just
WARN().

5f0dfb7a9b mpt3sas: Used "synchronize_irq()"API to synchronize timed-out IO & TMs
Replaced mpt3sas_base_flush_reply_queues() with
mpt3sas_base_sync_reply_irqs(),as mpt3sas_base_flush_reply_queues()
skips over reply queues that are currently busy (i.e. being handled by
interrupt processing in another core). If a reply queue is busy, then
call to synchronize_irq()in mpt3sas_base_sync_reply_irqs()make sures the
other core has finished flushing the queue and completed any calls to
the mid-layer scsi_done() routine.

In-order to handle this 'MPI2_EVENT_ACTIVE_CABLE_EXCEPTION' event,
driver need to follow below steps,
1. Unmask the 'MPI2_EVENT_ACTIVE_CABLE_EXCEPTION' event,
so that FW can notify this event to host driver.
2. After receiving this event, add this event to AEN event queue,
for notifying this event to applications.
3. Then Print below message in kernel logs if the event data's reason
code is zero,
"Currently an active cable with ReceptacleID <ID_Value> cannot be powered
and devices connected to this active cable will not be seen. This active
cable requires <PowerValue_in_mW> of power"

This event is only for Intruder/Cutlass HBAs.

'struct timeval' will have its tv_sec value overflow on 32-bit systems
in year 2038 and beyond. This patch replaces the use of struct timeval
for computing mpi_request.TimeStamp, and instead uses ktime_t which
provides 64-bit seconds value. The timestamp computed remains
unaffected (milliseconds since Unix epoch).

7f8b8f3fba mpt3sas: Remove unnecessary synchronize_irq() before free_irq()
Calling synchronize_irq() right before free_irq() is quite useless. On
one hand the IRQ can easily fire again before free_irq() is entered, on
the other hand free_irq() itself calls synchronize_irq() internally (in
a race condition free way), before any state associated with the IRQ is
freed.

Patch was generated using the following semantic patch:

Module parameter to enable/disable configuring affinity hint for msix
vector.  SMP affinity feature can be enabled/disabled by setting module
parameter "smp_affinity_enable" to 1/0.  By default this feature is
enabled. (smp_affinity_enable = 1 enabled).

Driver assumes HighPriority credit as part of Global credit. But,
Firmware treats HighPriority credit value and global cedits as two
different values. Changed host queue algorithm to treat global credits
and highPriority credits as two different values.
