6ce377afd1 xfs: fix reading of wrapped log data
Commit 4439647 ("xfs: reset buffer pointers before freeing them") in
3.0-rc1 introduced a regression when recovering log buffers that
wrapped around the end of log. The second part of the log buffer at
the start of the physical log was being read into the header buffer
rather than the data buffer, and hence recovery was seeing garbage
in the data buffer when it got to the region of the log buffer that
was incorrectly read.

03b1293eda xfs: fix buffer shudown reference count mismatch
When we shut down the filesystem, we have to unpin and free all the
buffers currently active in the CIL. To do this we unpin and remove
them in one operation as a result of a failed iclogbuf write. For
buffers, we do this removal via a simultated IO completion of after
marking the buffer stale.

At the time we do this, we have two references to the buffer - the
active LRU reference and the buf log item.  The LRU reference is
removed by marking the buffer stale, and the active CIL reference is
by the xfs_buf_iodone() callback that is run by
xfs_buf_do_callbacks() during ioend processing (via the bp->b_iodone
callback).

However, ioend processing requires one more reference - that of the
IO that it is completing. We don't have this reference, so we free
the buffer prematurely and use it after it is freed. For buffers
marked with XBF_ASYNC, this leads to assert failures in
xfs_buf_rele() on debug kernels because the b_hold count is zero.

Fix this by making sure we take the necessary IO reference before
starting IO completion processing on the stale buffer, and set the
XBF_ASYNC flag to ensure that IO completion processing removes all
the active references from the buffer to ensure it is fully torn
down.

ca250b1b3d xfs: invalidate allocbt blocks moved to the free list
When we free a block from the alloc btree tree, we move it to the
freelist held in the AGFL and mark it busy in the busy extent tree.
This typically happens when we merge btree blocks.

Once the transaction is committed and checkpointed, the block can
remain on the free list for an indefinite amount of time.  Now, this
isn't the end of the world at this point - if the free list is
shortened, the buffer is invalidated in the transaction that moves
it back to free space. If the buffer is allocated as metadata from
the free list, then all the modifications getted logged, and we have
no issues, either. And if it gets allocated as userdata direct from
the freelist, it gets invalidated and so will never get written.

However, during the time it sits on the free list, pressure on the
log can cause the AIL to be pushed and the buffer that covers the
block gets pushed for write. IOWs, we end up writing a freed
metadata block to disk. Again, this isn't the end of the world
because we know from the above we are only writing to free space.

The problem, however, is for validation callbacks. If the block was
on old btree root block, then the level of the block is going to be
higher than the current tree root, and so will fail validation.
There may be other inconsistencies in the block as well, and
currently we don't care because the block is in free space. Shutting
down the filesystem because a freed block doesn't pass write
validation, OTOH, is rather unfriendly.

So, make sure we always invalidate buffers as they move from the
free space trees to the free list so that we guarantee they never
get written to disk while on the free list.

1e7acbb7bc xfs: silence uninitialised f.file warning.
Uninitialised variable build warning introduced by 2903ff0 ("switch
simple cases of fget_light to fdget"), gcc is not smart enough to
work out that the variable is not used uninitialised, and the commit
removed the initialisation at declaration that the old variable had.

eaef854335 xfs: growfs: don't read garbage for new secondary superblocks
When updating new secondary superblocks in a growfs operation, the
superblock buffer is read from the newly grown region of the
underlying device. This is not guaranteed to be zero, so violates
the underlying assumption that the unused parts of superblocks are
zero filled. Get a new buffer for these secondary superblocks to
ensure that the unused regions are zero filled correctly.

1f3c785c3a xfs: move allocation stack switch up to xfs_bmapi_allocate
Switching stacks are xfs_alloc_vextent can cause deadlocks when we
run out of worker threads on the allocation workqueue. This can
occur because xfs_bmap_btalloc can make multiple calls to
xfs_alloc_vextent() and even if xfs_alloc_vextent() fails it can
return with the AGF locked in the current allocation transaction.

If we then need to make another allocation, and all the allocation
worker contexts are exhausted because the are blocked waiting for
the AGF lock, holder of the AGF cannot get it's xfs-alloc_vextent
work completed to release the AGF.  Hence allocation effectively
deadlocks.

To avoid this, move the stack switch one layer up to
xfs_bmapi_allocate() so that all of the allocation attempts in a
single switched stack transaction occur in a single worker context.
This avoids the problem of an allocation being blocked waiting for
a worker thread whilst holding the AGF.

326c03555b xfs: introduce XFS_BMAPI_STACK_SWITCH
Certain allocation paths through xfs_bmapi_write() are in situations
where we have limited stack available. These are almost always in
the buffered IO writeback path when convertion delayed allocation
extents to real extents.

The current stack switch occurs for userdata allocations, which
means we also do stack switches for preallocation, direct IO and
unwritten extent conversion, even those these call chains have never
been implicated in a stack overrun.

Hence, let's target just the single stack overun offended for stack
switches. To do that, introduce a XFS_BMAPI_STACK_SWITCH flag that
the caller can pass xfs_bmapi_write() to indicate it should switch
stacks if it needs to do allocation.

7e9620f21d xfs: only update the last_sync_lsn when a transaction completes
The log write code stamps each iclog with the current tail LSN in
the iclog header so that recovery knows where to find the tail of
thelog once it has found the head. Normally this is taken from the
first item on the AIL - the log item that corresponds to the oldest
active item in the log.

The problem is that when the AIL is empty, the tail lsn is dervied
from the the l_last_sync_lsn, which is the LSN of the last iclog to
be written to the log. In most cases this doesn't happen, because
the AIL is rarely empty on an active filesystem. However, when it
does, it opens up an interesting case when the transaction being
committed to the iclog spans multiple iclogs.

That is, the first iclog is stamped with the l_last_sync_lsn, and IO
is issued. Then the next iclog is setup, the changes copied into the
iclog (takes some time), and then the l_last_sync_lsn is stamped
into the header and IO is issued. This is still the same
transaction, so the tail lsn of both iclogs must be the same for log
recovery to find the entire transaction to be able to replay it.

The problem arises in that the iclog buffer IO completion updates
the l_last_sync_lsn with it's own LSN. Therefore, If the first iclog
completes it's IO before the second iclog is filled and has the tail
lsn stamped in it, it will stamp the LSN of the first iclog into
it's tail lsn field. If the system fails at this point, log recovery
will not see a complete transaction, so the transaction will no be
replayed.

The fix is simple - the l_last_sync_lsn is updated when a iclog
buffer IO completes, and this is incorrect. The l_last_sync_lsn
shoul dbe updated when a transaction is completed by a iclog buffer
IO. That is, only iclog buffers that have transaction commit
callbacks attached to them should update the l_last_sync_lsn. This
means that the last_sync_lsn will only move forward when a commit
record it written, not in the middle of a large transaction that is
rolling through multiple iclog buffers.

Right, tmpfs is being stupid to access fid->raw[2] before validating that
fh_len includes it: the buffer kmalloc'ed by do_sys_name_to_handle() may
fall at the end of a page, and the next page not be present.

But some other filesystems (ceph, gfs2, isofs, reiserfs, xfs) are being
careless about fh_len too, in fh_to_dentry() and/or fh_to_parent(), and
could oops in the same way: add the missing fh_len checks to those.

8c0a853770 fs: push rcu_barrier() from deactivate_locked_super() to filesystems
There's no reason to call rcu_barrier() on every
deactivate_locked_super().  We only need to make sure that all delayed rcu
free inodes are flushed before we destroy related cache.

Removing rcu_barrier() from deactivate_locked_super() affects some fast
paths.  E.g.  on my machine exit_group() of a last process in IPC
namespace takes 0.07538s.  rcu_barrier() takes 0.05188s of that time.

 "This is a mostly modest set of changes to enable basic user namespace
  support.  This allows the code to code to compile with user namespaces
  enabled and removes the assumption there is only the initial user
  namespace.  Everything is converted except for the most complex of the
  filesystems: autofs4, 9p, afs, ceph, cifs, coda, fuse, gfs2, ncpfs,
  nfs, ocfs2 and xfs as those patches need a bit more review.

  The strategy is to push kuid_t and kgid_t values are far down into
  subsystems and filesystems as reasonable.  Leaving the make_kuid and
  from_kuid operations to happen at the edge of userspace, as the values
  come off the disk, and as the values come in from the network.
  Letting compile type incompatible compile errors (present when user
  namespaces are enabled) guide me to find the issues.

  The most tricky areas have been the places where we had an implicit
  union of uid and gid values and were storing them in an unsigned int.
  Those places were converted into explicit unions.  I made certain to
  handle those places with simple trivial patches.

  Out of that work I discovered we have generic interfaces for storing
  quota by projid.  I had never heard of the project identifiers before.
  Adding full user namespace support for project identifiers accounts
  for most of the code size growth in my git tree.

  Ultimately there will be work to relax privlige checks from
  "capable(FOO)" to "ns_capable(user_ns, FOO)" where it is safe allowing
  root in a user names to do those things that today we only forbid to
  non-root users because it will confuse suid root applications.

  While I was pushing kuid_t and kgid_t changes deep into the audit code
  I made a few other cleanups.  I capitalized on the fact we process
  netlink messages in the context of the message sender.  I removed
  usage of NETLINK_CRED, and started directly using current->tty.

  Some of these patches have also made it into maintainer trees, with no
  problems from identical code from different trees showing up in
  linux-next.

  After reading through all of this code I feel like I might be able to
  win a game of kernel trivial pursuit."

Pull workqueue changes from Tejun Heo:
 "This is workqueue updates for v3.7-rc1.  A lot of activities this
  round including considerable API and behavior cleanups.

   * delayed_work combines a timer and a work item.  The handling of the
     timer part has always been a bit clunky leading to confusing
     cancelation API with weird corner-case behaviors.  delayed_work is
     updated to use new IRQ safe timer and cancelation now works as
     expected.

   * Another deficiency of delayed_work was lack of the counterpart of
     mod_timer() which led to cancel+queue combinations or open-coded
     timer+work usages.  mod_delayed_work[_on]() are added.

     These two delayed_work changes make delayed_work provide interface
     and behave like timer which is executed with process context.

   * A work item could be executed concurrently on multiple CPUs, which
     is rather unintuitive and made flush_work() behavior confusing and
     half-broken under certain circumstances.  This problem doesn't
     exist for non-reentrant workqueues.  While non-reentrancy check
     isn't free, the overhead is incurred only when a work item bounces
     across different CPUs and even in simulated pathological scenario
     the overhead isn't too high.

     All workqueues are made non-reentrant.  This removes the
     distinction between flush_[delayed_]work() and
     flush_[delayed_]_work_sync().  The former is now as strong as the
     latter and the specified work item is guaranteed to have finished
     execution of any previous queueing on return.

   * In addition to the various bug fixes, Lai redid and simplified CPU
     hotplug handling significantly.

   * Joonsoo introduced system_highpri_wq and used it during CPU
     hotplug.

  There are two merge commits - one to pull in IRQ safe timer from
  tip/timers/core and the other to pull in CPU hotplug fixes from
  wq/for-3.6-fixes as Lai's hotplug restructuring depended on them."

Fixed a number of trivial conflicts, but the more interesting conflicts
were silent ones where the deprecated interfaces had been used by new
code in the merge window, and thus didn't cause any real data conflicts.

Tejun pointed out a few of them, I fixed a couple more.

2d2194f61f xfs: reduce code duplication handling inode32/64 options
Add xfs_set_inode32() to be used to enable inode32 allocation mode. this
will reduce the amount of duplicated code needed to mount/remount a
filesystem with inode32 option.  This patch also changes
xfs_set_inode64() to return the maximum AG number that inodes can be
allocated instead of set mp->m_maxagi by itself, so that the behaviour
is the same as xfs_set_inode32().  This simplifies code that calls these
functions and needs to know the maximum AG that inodes can be allocated
in.

08bf540412 xfs: make inode64 as the default allocation mode
since 64-bit inodes can be accessed while using inode32, and these can
also be used on 32-bit kernels, there is no reason to still keep inode32
as the default mount option.  If the filesystem cannot handle 64bit
inode numbers (i.e CONFIG_LBDAF is not enabled and BITS_PER_LONG == 32),
XFS_MOUNT_SMALL_INUMS will still be set by default, so inode64 is not an
unconditional default value.

8aea3ff411 xfs: Fix m_agirotor reset during AG selection
xfs_ialloc_next_ag() currently resets m_agirotor when it is equal to
m_maxagi:

         if (++mp->m_agirotor == mp->m_maxagi)
	         mp->m_agirotor = 0;

But, if for some reason mp->m_maxagi changes to a lower value than
current m_agirotor, this condition will never be true, causing
m_agirotor to exceed the maximum allowed value (m_maxagi).

This implies mainly during lookups for xfs_perag structs in its radix
tree, since the agno value used for the lookup is based on m_agirotor.
An out-of-range m_agirotor may cause a lookup failure which in case will
return NULL.

As an example, the value of m_maxagi is decreased during
inode64->inode32 remount process, case where I've found this problem.

431f19744d userns: Convert quota netlink aka quota_send_warning
Modify quota_send_warning to take struct kqid instead a type and
identifier pair.

When sending netlink broadcasts always convert uids and quota
identifiers into the intial user namespace.  There is as yet no way to
send a netlink broadcast message with different contents to receivers
in different namespaces, so for the time being just map all of the
identifiers into the initial user namespace which preserves the
current behavior.

Change the callers of quota_send_warning in gfs2, xfs and dquot
to generate a struct kqid to pass to quota send warning.  When
all of the user namespaces convesions are complete a struct kqid
values will be availbe without need for conversion, but a conversion
is needed now to avoid needing to convert everything at once.

74a8a10378 userns: Convert qutoactl
Update the quotactl user space interface to successfull compile with
user namespaces support enabled and to hand off quota identifiers to
lower layers of the kernel in struct kqid instead of type and qid
pairs.

The quota on function is not converted because while it takes a quota
type and an id.  The id is the on disk quota format to use, which
is something completely different.

The signature of two struct quotactl_ops methods were changed to take
struct kqid argumetns get_dqblk and set_dqblk.

The dquot, xfs, and ocfs2 implementations of get_dqblk and set_dqblk
are minimally changed so that the code continues to work with
the change in parameter type.

This is the first in a series of changes to always store quota
identifiers in the kernel in struct kqid and only use raw type and qid
values when interacting with on disk structures or userspace.  Always
using struct kqid internally makes it hard to miss places that need
conversion to or from the kernel internal values.

5f3a4a28ec userns: Pass a userns parameter into posix_acl_to_xattr and posix_acl_from_xattr
- Pass the user namespace the uid and gid values in the xattr are stored
   in into posix_acl_from_xattr.

 - Pass the user namespace kuid and kgid values should be converted into
   when storing uid and gid values in an xattr in posix_acl_to_xattr.

- Modify all callers of posix_acl_from_xattr and posix_acl_to_xattr to
  pass in &init_user_ns.

In the short term this change is not strictly needed but it makes the
code clearer.  In the longer term this change is necessary to be able to
mount filesystems outside of the initial user namespace that natively
store posix acls in the linux xattr format.

While xfs_buftarg_shrink() is freeing buffers from the dispose list (filled with
buffers from lru list), there is a possibility to have xfs_buf_stale() racing
with it, and removing buffers from dispose list before xfs_buftarg_shrink() does
it.

This happens because xfs_buftarg_shrink() handle the dispose list without
locking and the test condition in xfs_buf_stale() checks for the buffer being in
*any* list:

if (!list_empty(&bp->b_lru))

If the buffer happens to be on dispose list, this causes the buffer counter of
lru list (btp->bt_lru_nr) to be decremented twice (once in xfs_buftarg_shrink()
and another in xfs_buf_stale()) causing a wrong account usage of the lru list.

This may cause xfs_buftarg_shrink() to return a wrong value to the memory
shrinker shrink_slab(), and such account error may also cause an underflowed
value to be returned; since the counter is lower than the current number of
items in the lru list, a decrement may happen when the counter is 0, causing
an underflow on the counter.

The fix uses a new flag field (and a new buffer flag) to serialize buffer
handling during the shrink process. The new flag field has been designed to use
btp->bt_lru_lock/unlock instead of xfs_buf_lock/unlock mechanism.

dchinner, sandeen, aquini and aris also deserve credits for this.

43829731dd workqueue: deprecate flush[_delayed]_work_sync()
flush[_delayed]_work_sync() are now spurious.  Mark them deprecated
and convert all users to flush[_delayed]_work().

If you're cc'd and wondering what's going on: Now all workqueues are
non-reentrant and the regular flushes guarantee that the work item is
not pending or running on any CPU on return, so there's no reason to
use the sync flushes at all and they're going away.

This patch doesn't make any functional difference.

1ed845df60 xfs: kill struct declarations in xfs_mount.h
I noticed that "struct xfs_mount_args" was still declared in
"fs/xfs/xfs_mount.h".  That struct doesn't even exist any more (and
is obviously not referenced elsewhere in that header file).  While
in there, delete four other unneeded struct declarations in that
file.

Doing so highlights that "fs/xfs/xfs_trace.h" was relying indirectly
on "xfs_mount.h" to be #included in order to declare "struct
xfs_bmbt_irec", so add that declaration to resolve that issue.

d9457dc056 xfs: Convert to new freezing code
Generic code now blocks all writers from standard write paths. So we add
blocking of all writers coming from ioctl (we get a protection of ioctl against
racing remount read-only as a bonus) and convert xfs_file_aio_write() to a
non-racy freeze protection. We also keep freeze protection on transaction
start to block internal filesystem writes such as removal of preallocated
blocks.

9a57fa8ee7 xfs: wait for the write the superblock on unmount
v2: Add the xfs_buf_lock to xfs_quiesce_attr().
    Add explaination why xfs_buf_lock() is used to wait for write.

xfs_wait_buftarg() does not wait for the completion of the write of the
uncached superblock. This write can race with the shutdown of the log
and causes a panic if the write does not win the race.

During the log write, xfsaild_push() will lock the buffer and set the
XBF_ASYNC flag. Because the XBF_FLAG is set, complete() is not performed
on the buffer's iowait entry, we cannot call xfs_buf_iowait() to wait
for the write to complete. The buffer's lock is held until the write is
complete, so we can block on a xfs_buf_lock() request to be notified
that the write is complete.
