When aborting in-flight request for recovering controller, we have
to make sure that queue's complete function is called on completed
request before moving on. Otherwise, for example, the warning of
WARN_ON_ONCE(qp->mrs_used > 0) in ib_destroy_qp_user() may be
triggered on nvme-rdma.
Fix this issue by using blk_mq_tagset_wait_completed_request.

allocated to make mq more efficient by reducing the size of the
preallocated sg list. This necessitates a large number of driver
changes because the previous guarantee that if a driver specified
SG_ALL as the size of its scatter list, it would get a non-chained
list and didn't need to bother with scatterlist iterators is now
broken and every driver *must* use scatterlist iterators.
This was broken out as a separate topic because we need to convert all
the drivers before pulling the trigger and unconverted drivers kept
being found, necessitating a rebase.

sg_alloc_table_chained() currently allows the caller to provide one
preallocated SGL and returns if the requested number isn't bigger than
size of that SGL. This is used to inline an SGL for an IO request.
However, scattergather code only allows that size of the 1st preallocated
SGL to be SG_CHUNK_SIZE(128). This means a substantial amount of memory
(4KB) is claimed for the SGL for each IO request. If the I/O is small, it
would be prudent to allocate a smaller SGL.
Introduce an extra parameter to sg_alloc_table_chained() and
sg_free_table_chained() for specifying size of the preallocated SGL.
Both __sg_free_table() and __sg_alloc_table() assume that each SGL has the
same size except for the last one. Change the code to allow both functions
to accept a variable size for the 1st preallocated SGL.

The reason for this crash is accessing an already freed ib_device for
performing dma_unmap during exit_request commands. The root cause for
that is that during re-connection all the queues are destroyed and
re-created (and the ib_device is reference counted by the queues and
freed as well) but the tagset stays alive and all the DMA mappings (that
we perform in init_request) kept in the request context. The original
commit fixed a different bug that was introduced during bonding (aka nic
teaming) tests that for some scenarios change the underlying ib_device
and caused memory leakage and possible segmentation fault. This commit
is a complementary commit that also changes the wrong DMA mappings that
were saved in the request context and making the request sqe dma
mappings dynamic with the command lifetime (i.e. mapped in .queue_rq and
unmapped in .complete). It also fixes the above crash of accessing freed
ib_device during destruction of the tagset.

When the controller supports less queues than requested, we
should make sure that queue mapping does the right thing and
not assume that all queues are available. This fixes a crash
when the controller supports less queues than requested.
The rules are:
1. if no write/poll queues are requested, we assign the available queues
to the default queue map. The default and read queue maps share the
existing queues.
2. if write queues are requested:
- first make sure that read queue map gets the requested
nr_io_queues count
- then grant the default queue map the minimum between the requested
nr_write_queues and the remaining queues. If there are no available
queues to dedicate to the default queue map, fallback to (1) and
share all the queues in the existing queue map.
3. if poll queues are requested:
- map the remaining queues to the poll queue map.
Also, provide a log indication on how we constructed the different
queue maps.

In the past, before adding f41725bb ("nvme-rdma: Use mr pool") commit,
we needed a reference on the ib_device as long as the tagset
was alive, as the MRs in the request structures needed a valid ib_device.
Now, we allocate/deallocate MR pool per QP and consume on demand.
Also remove nvme_rdma_free_tagset function and use blk_mq_free_tag_set
instead, as it unneeded anymore.
This commit also fixes a memory leakage and possible segmentation fault.
When configuring the system with NIC teaming (aka bonding), we use 1
network interface to create an HA connection to the target side. In case
one connection breaks down, nvme-rdma driver will get notification from
rdma-cm layer that underlying address was change and will start error
recovery process. During this process, we'll reconnect to the target
via the second interface in the bond without destroying the tagset.
This will cause a leakage of the initial rdma device (ndev) and miscount
in the reference count of the new created rdma device (new ndev). In
the final destruction (or in another error flow), we'll get a warning
dump from the ib_dealloc_pd that we still have inflight MR's related to
that pd. This happens becasue of the miscount of the reference tag of
the rdma device and causing access violation to it's elements (some
queues are not destroyed yet).

If we timeout the admin startup sequence we might not yet have
an I/O tagset allocated which causes the teardown sequence to crash.
Make nvme_tcp_teardown_io_queues safe by not iterating inflight tags
if the tagset wasn't allocated.

Use blk_rq_nr_phys_segments() instead of blk_rq_payload_bytes() to check
if a command contains data to be mapped. This fixes the case where
a struct request contains LBAs, but it has no payload, such as
Write Zeroes support.

If the device supports less queues than provided (if the device has less
completion vectors), we might hit a bug due to the fact that we ignore
that in nvme_rdma_map_queues (we override the maps nr_queues with user
opts).
Instead, keep track of how many default/read/poll queues we actually
allocated (rather than asked by the user) and use that to assign our
queue mappings.

Currently, we have several problems with the timeout
handler:
1. If we timeout on the controller establishment flow, we will hang
because we don't execute the error recovery (and we shouldn't because
the create_ctrl flow needs to fail and cleanup on its own)
2. We might also hang if we get a disconnet on a queue while the
controller is already deleting. This racy flow can cause the controller
disable/shutdown admin command to hang.
We cannot complete a timed out request from the timeout handler without
mutual exclusion from the teardown flow (e.g. nvme_rdma_error_recovery_work).
So we serialize it in the timeout handler and teardown io and admin
queues to guarantee that no one races with us from completing the
request.

When passed with nr_poll_queues setup additional queues with cq polling
context IB_POLL_DIRECT (no interrupts) and make sure to set
QUEUE_FLAG_POLL on the connect_q. In addition add the third queue
mapping for polling queues.
nvmf connect on this queue is polled for like all other requests so make
nvmf_connect_io_queue poll for polling queues.

Instead of directly poking into the struct device add a new numa_node
field to struct nvme_ctrl. This allows fabrics drivers where ctrl->dev
is a virtual device to support NUMA affinity as well.
Also expose the field as a sysfs attribute, and populate it for the
RDMA and FC transports.

If we want to support async IO polling, then we have to allow finding
completions that aren't just for the one we are looking for. Always pass
in -1 to the mq_ops->poll() helper, and have that return how many events
were found in this poll loop.

Centralize controller sequence to a single routine that correctly cleans
up after failures instead of having multiple apperances in several flows
(create, reset, reconnect).
One thing that we also gain here are the sanity/boundary checks also
when connecting back to a dynamic controller.

If the controller is going away, we need to unquiesce the IO queues so
that all pending request can fail gracefully before moving forward with
controller deletion. Do that before we destroy the IO queues so
blk_cleanup_queue won't block in freeze.

The revised if_ready checks skipped over the case of returning error when
the controller is being deleted. Instead it was returning BUSY, which
caused the ios to retry, which caused the ns delete to hang waiting for

Allow up to 4 segments of inline data for NVMF WRITE operations. This
reduces latency for small WRITEs by removing the need for the target to
issue a READ WR for IB, or a REG_MR + READ WR chain for iWarp.
Also cap the inline segments used based on the limitations of the
device.

If reconnect/reset failed where the controller async event buffer
was freed, we might end up freeing it again as we call
nvme_rdma_destroy_admin_queue again in the remove path. Given that
the sequence is guaranteed to serialize by .ctrl_stop, we simply
set ctrl->async_event_sqe.data to NULL and don't free it in future
visits.

That is user argument, and theoretically controller limits can change
over time (over reconnects/resets). Instead, use the sqsize controller
attribute to check queue depth boundaries and use it to the tagset
allocation.

The race is between completing the request at error recovery work and
rdma completions. If we cancel the request before getting the good
rdma completion we get a NULL deref of the request MR at
nvme_rdma_process_nvme_rsp().
When Canceling the request we return its mr to the mr pool (set mr to
NULL) and also unmap its data. Canceling the requests while the rdma
queues are active is not safe. Because rdma queues are active and we
get good rdma completions that can use the mr pointer which may be NULL.
Completing the request too soon may lead also to performing DMA to/from
user buffers which might have been already unmapped.
The commit fixes the race by draining the QP before starting the abort
commands mechanism.

If nvme_rdma_configure_admin_queue fails before we allocated
the async event buffer, we will falsly free it because
nvme_rdma_free_queue is freeing it. Fix it by allocating the buffer right
after nvme_rdma_alloc_queue and free it right before nvme_rdma_queue_free
to maintain orderly reverse cleanup sequence.

Failures after nvme_init_ctrl will defer resource cleanups to .free_ctrl
when the reference is released, hence we should not free the controller
queues for these failures.
Fix that by moving controller queues allocation before controller
initialization and correctly freeing them for failures before
initialization and skip them for failures after initialization.

Move the is_connected check to the fibre channel transport, as it has no
meaning for other transports. To facilitate this split out a new
nvmf_fail_nonready_command helper that is called by the transport when
it is asked to handle a command on a queue that is not ready.
Also avoid a function call for the queue live fast path by inlining
the check.

For any failure after nvme_rdma_start_queue in
nvme_rdma_configure_admin_queue, the admin queue will be freed with the
NVME_RDMA_Q_LIVE flag still set. Once nvme_rdma_stop_queue is invoked,
that will cause a use-after-free.

The nvmf_check_if_ready() checks that were added are very simplistic.
As such, the routine allows a lot of cases to fail ios during windows
of reset or re-connection. In cases where there are not multi-path
options present, the error goes back to the callee - the filesystem
or application. Not good.
The common routine was rewritten and calling syntax slightly expanded
so that per-transport is_ready routines don't need to be present.
The transports now call the routine directly. The routine is now a
fabrics routine rather than an inline function.
The routine now looks at controller state to decide the action to
take. Some states mandate io failure. Others define the condition where
a command can be accepted. When the decision is unclear, a generic
queue-or-reject check is made to look for failfast or multipath ios and
only fails the io if it is so marked. Otherwise, the io will be queued
and wait for the controller state to resolve.
Admin commands issued via ioctl share a live admin queue with commands
from the transport for controller init. The ioctls could be intermixed
with the initialization commands. It's possible for the ioctl cmd to
be issued prior to the controller being enabled. To block this, the
ioctl admin commands need to be distinguished from admin commands used
for controller init. Added a USERCMD nvme_req(req)->rq_flags bit to
reflect this division and set it on ioctls requests. As the
nvmf_check_if_ready() routine is called prior to nvme_setup_cmd(),
ensure that commands allocated by the ioctl path (actually anything
in core.c) preps the nvme_req(req) before starting the io. This will
preserve the USERCMD flag during execution and/or retry.

For consistancy reasons, any fabric-specific works
(e.g error recovery/reconnect) should be canceled in
nvme_stop_ctrl, as for all other NVMe pending works
(e.g. scan, keep alive).
The patch aims to simplify the logic of the code, as
we now only rely on a vague demand from any fabric
to flush its private workqueues at the beginning of
.delete_ctrl op.

When reset_controller that is invoked by sysfs fails,
it enters an error flow which practically removes the
nvme ctrl entirely (similar to delete_ctrl flow). It
causes the system to hang, since a sysfs attribute cannot
be unregistered by one of its own methods.
This can be fixed by calling delete_ctrl as a work rather
than sequential code. In addition, it should give the ctrl
a chance to recover using reconnection mechanism (consistant
with FC reset_ctrl error flow). Also, while we're here, return
suitable errno in case the reset ended with non live ctrl.

In order to avoid concurrent error recovery during initialization
process (allowed by the NVME_CTRL_NEW --> NVME_CTRL_RESETTING transition)
we must mark the ctrl as CONNECTING before initial connection
establisment.

NVMe transport driver module unload may (and usually does) trigger
iteration over the active controllers and delete them all (sometimes
under a mutex). However, a controller can be created concurrently with
module unload which can lead to leakage of resources (most important char
device node leakage) in case the controller creation occured after the
unload delete and drain sequence. To protect against this, we take a
module reference to guarantee that the nvme transport driver is not
unloaded while creating a controller.

Now ctrl state machine allows to transition from RESETTING to
RECONNECTING. In nvme-rdma when we receive a rdma cm DISONNECTED event,
we trigger nvme_rdma_error_recovery. This happens also when we execute a
controller reset, issue a cm diconnect request and receive a cm
disconnect reply, as a result, the reset work and the error recovery work
can run concurrently.
Until now the state machine prevented from the error recovery work from
running as a result of a controller reset (RESETTING -> RECONNECTING was
not allowed).
To fix this, we adopt the FC state machine approach, we always transition
from LIVE to RESETTING and only then to RECONNECTING. We do this both
for the error recovery work and the controller reset work:
1. transition to RESETTING
2. teardown the controller association
3. transition to RECONNECTING
This will restore the protection against reset work and error recovery work
from concurrently running together.

In case nvme_rdma_wait_for_cm timeout expires before we get
an established or rejected event (rdma_connect succeeded) from
rdma_cm, we end up with leaking the ib transport resources for
dedicated queue. This scenario can easily reproduced using traffic
test during port toggling.
Also, in order to protect from parallel ib queue destruction, that
may be invoked from different context's, introduce new flag that
stands for transport readiness. While we're here, protect also against
a situation that we can receive rdma_cm events during ib queue destruction.

Currently, blk_mq_tagset_iter() iterate over initial hctx tags only. If
an I/O scheduler is used, it doesn't iterate the hctx scheduler tags and
the static request aren't been updated. For example, while using NVMe
over Fabrics RDMA host, this cause us not to reinit the scheduler
requests and thus not re-register all the memory regions during the
tagset re-initialization in the reconnect flow.
This may lead to a memory registration error:
"MEMREG for CQE 0xffff88044c14dce8 failed with status memory management operation error (6)"
With this commit we don't need to reinit the requests, and thus fix this
failure.

We must not complete a request before the host memory region is
invalidated. Luckily we have send with invalidate protocol support so
we usually don't need to execute it, but in case the target did not
invalidate a memory region for us, we must wait for the invalidation to
complete before unmapping host memory and completing the I/O.

In order to guarantee that the HCA will never get an access violation
(either from invalidated rkey or from iommu) when retrying a send
operation we must complete a request only when both send completion and
the nvme cqe has arrived. We need to set the send/recv completions flags
atomically because we might have more than a single context accessing the
request concurrently (one is cq irq-poll context and the other is
user-polling used in IOCB_HIPRI).
Only then we are safe to invalidate the rkey (if needed), unmap the host
buffers, and complete the IO.

The entire completions suppress mechanism is currently broken because the
HCA might retry a send operation (due to dropped ack) after the nvme
transaction has completed.
In order to handle this, we signal all send completions and introduce a
separate done handler for async events as they will be handled differently
(as they don't include in-capsule data by definition).

When the fabrics queue is not alive and fully functional, no commands
should be allowed to pass but connect (which moves the queue to a fully
functional state). Any other command should be failed, with either
temporary status BLK_STS_RESOUCE or permanent status BLK_STS_IOERR.
This is shared across all fabrics, hence move the check to fabrics
library.

Adds support for the duplicate_connect option. When set to true,
checks whether there's an existing controller via the same target
address (traddr), target port (trsvcid), and if specified, host
address (host_traddr). Fails the connection request if there is
an existing controller.

Instead of allocating a separate struct device for the character device
handle embedd it into struct nvme_ctrl and use it for the main controller
refcounting. This removes double refcounting and gets us an automatic
reference for the character device operations. We keep ctrl->device as a
pointer for now to avoid chaning printks all over, but in the future we
could look into message printing helpers that take a controller structure
similar to what other subsystems do.
Note the delete_ctrl operation always already has a reference (either
through sysfs due this change, or because every open file on the
/dev/nvme-fabrics node has a refernece) when it is entered now, so we
don't need to do the unless_zero variant there.

nvme_rdma_queue_is_ready() fails requests in case a queue is not
LIVE. If the controller is in RECONNECTING state, we might be in
this state for a long time (until we successfully reconnect) and
we are better off with failing the request fast. Otherwise, we
fail with BLK_STS_RESOURCE to have the block layer try again
soon.
In case we are removing the controller when the admin queue
is not LIVE, we will terminate the request with BLK_STS_RESOURCE
but it happens before we call blk_mq_start_request() so the
request timeout never expires, and the queue will never get
back to LIVE (because we are removing the controller). This
causes the removal operation to block infinitly [1].
Thus, if we are removing (state DELETING), and the queue is
not LIVE, we need to fail the request permanently as there is
no chance for it to ever complete successfully.

The fact that we free the async event buffer in
nvme_rdma_destroy_admin_queue can cause us to free it
more than once because this happens in every reconnect
attempt since commit 31fdf18401. we rely on the queue
state flags DELETING to avoid this for other resources.
A more complete fix is to not destroy the admin/io queues
unconditionally on every reconnect attempt, but its a bit
more extensive and will go in the next release.

No chance for the local invalidate to succeed if the queue-pair
is in error state. Most likely the target will do a remote
invalidation of our mr so not a big loss on the test_bit.

Relying on the queue state while tearing down on every reconnect

Move blk_mq_reinit_tagset from blk-mq to nvme core
as the only user of it. Current transports that use
it (rdma, fc) simply implement .reinit_request op.
This patch does not change any functionality.

By calling nvme_stop_ctrl on a already failed controller will wait for the
scan work to complete (only by identify timeout expiration which is 60
seconds). This is unnecessary when we already know that the controller has
failed.
