The #ifdef check around the definition doesn't match the one around the
declaration, leading to a link failure when CONFIG_XEN_DOM0 is enabled
but CONFIG_XEN_PV_DOM0 is not:

Change the declaration to use the same check that was already present
around the function definition.

The IOPL emulation via the I/O bitmap is sufficient. Remove the legacy
cruft dealing with the (e)flags based IOPL mechanism.

Build cpu_hotplug for ARM and ARM64 guests.
Rename arch_(un)register_cpu to xen_(un)register_cpu and provide an
empty implementation on ARM and ARM64. On x86 just call
arch_(un)register_cpu as we are already doing.
Initialize cpu_hotplug on ARM.

This patch is similiar to Gleb Natapov's patch for KVM, which enable the
hypervisor to emulate x2apic feature for the guest. By this way, the emulation
of lapic would be simpler with x2apic interface(MSR), and faster.

When testing for a dom0/initial/privileged domain, make sure the
predicate evaluates to a compile-time 0 if CONFIG_XEN_DOM0 isn't
enabled. This will make most of the dom0 code evaporate without
much more effort.
