The endwin irq indicates that DSS has finished scanning out a buffer.
Use this to trigger page-flip event to userspace, so this happens
only *after* the previous buffer is finished.

When kernel reboot, tty circular buffer is reset before last TX DMA interrupt is called,
while the buffer tail is updated in TX DMA interrupt handler. So, don't update the buffer
tail if it is reset.

Non-hub device has no child, and even a real USB hub has ports far
less than USB_MAXCHILDREN, so there is no need using a fix array for
child devices, just allocate it dynamically according real port
number.

Here's my final pull request for 3.4. All the patches have been under
review for some time (months in some cases). The ring expansion patches
in particular have been tested by both me and Paul Zimmerman from
Synopsis.
They add support for:
- Dynamic ring expansion
- New USB 2.1 link PM errata (BESL)
- xHCI host controller support for the Synopsis DesignWare 3 IP
The dynamic ring expansion patches finally make test 10 of the host-side
test pass, instead of failing due to no room on the endpoint ring for
the larger transfers. I would have hoped that the ring expansion
patchset would make the Point Grey USB 3.0 camera work, but sadly it
fails to respond to a control transfer on my test system. This doesn't
seem to be a driver bug, but it could be a device or host bug.
Felipe has tested the patches to add a platform device to the xHCI
driver on the Synopsis DesignWare 3 IP in the TI OMAP5 board.

As described in e6fa16ab ("signal: sigprocmask() should do
retarget_shared_pending()") the modification of current->blocked is
incorrect as we need to check whether the signal we're about to block
is pending in the shared queue.
Also, use the new helper function introduced in commit 5e6292c0f2
("signal: add block_sigmask() for adding sigmask to current->blocked")
which centralises the code for updating current->blocked after
successfully delivering a signal and reduces the amount of duplicate
code across architectures. In the past some architectures got this
code wrong, so using this helper function should stop that from
happening again.

When a urb is submitted to xHCI driver, check if queueing the urb will make
the enqueue pointer advance into dequeue seg and expand the ring if it
occurs. This is to guarantee the safety of ring expansion.

If room_on_ring() check fails, try to expand the ring and check again.
When expand a ring, use a cached ring or allocate new segments, link
the original ring and the new ring or segments, update the original ring's
segment numbers and the last segment pointer.

In the past all the rings were allocated with cycle state equal to 1.
Now the driver may expand an existing ring, and the new segments shall be
allocated with the same cycle state as the old one.
This affects ring allocation and cached ring re-initialization.

In the past, the room_on_ring() check was implemented by walking all over
the ring, which is wasteful and complicated.
Count the number of free TRBs instead. The free TRBs number should be
updated when enqueue/dequeue pointer is updated, or upon the completion
of a set dequeue pointer command.

This patch fixes a buffer overrun bug in
tracepoint_id_to_path(). The bug manisfested itself as a memory
error reported by perf record. I ran into it with perf sched:
$ perf sched rec noploop 2 noploop for 2 seconds
[ perf record: Woken up 14 times to write data ]
[ perf record: Captured and wrote 42.701 MB perf.data (~1865622 samples) ]
Fatal: No memory to alloc tracepoints list
It turned out that tracepoint_id_to_path() was reading the
tracepoint id using read() but the buffer was not large enough
to include the \n terminator for id with 4 digits or more.
The patch fixes the problem by extending the buffer to a more
reasonable size covering all possible id length include \n
terminator. Note that atoll() stops at the first non digit
character, thus it is not necessary to clear the buffer between
each read.

When a machine boots up, the TSC generally gets reset. However,
when kexec is used to boot into a kernel, the TSC value would be
carried over from the previous kernel. The computation of
cycns_offset in set_cyc2ns_scale is prone to an overflow, if the
machine has been up more than 208 days prior to the kexec. The
overflow happens when we multiply *scale, even though there is
enough room to store the final answer.
We fix this issue by decomposing tsc_now into the quotient and
remainder of division by CYC2NS_SCALE_FACTOR and then performing
the multiplication separately on the two components.
Refactor code to share the calculation with the previous
fix in __cycles_2_ns().

FCoE statistics ids were distinguished from the L2's statistics ids.
However, not all of the change was committed. This causes a possible
collision of indices when FCoE is present.
This patch fixes the issue.

When allocate a ring, store its type - four transfer types for endpoint,
TYPE_STREAM for stream transfer, and TYPE_COMMAND/TYPE_EVENT for xHCI host.
This helps to get rid of three bool function parameters: link_trbs, isoc
and consumer.

Various people reported nohz load tracking still being wrecked, but Doug
spotted the actual problem. We fold the nohz remainder in too soon,
causing us to loose samples and under-account.
So instead of playing catch-up up-front, always do a single load-fold
with whatever state we encounter and only then fold the nohz remainder
and play catch-up.

There's a few awkward printk()s inside of scheduler guts that people
prefer to keep but really are rather deadlock prone. Fudge around it
by storing the text in a per-cpu buffer and poll it using the existing
printk_tick() handler.
This will drop output when its more frequent than once a tick, however
only the affinity thing could possible go that fast and for that just
one should suffice to notify the admin he's done something silly..

The 'next_balance' field of 'nohz' idle balancer must be initialized
to jiffies. Since jiffies is initialized to negative 300 seconds the
'nohz' idle balancer does not run for the first 300s (5mins) after
bootup. If no new processes are spawed or no idle cycles happen, the
load on the cpus will remain unbalanced for that duration.
