c0ec382e19 x86/RAS: Remove mce.usable_addr
It is useless and we can use the function instead. Besides,
mcelog(8) hasn't managed to make use of it yet. So kill it.

7e0abcd6b7 x86/mce: Include linux/ioctl.h in uapi mce header
asm/ioctls.h contains definition for termios, not just the _IO* macros.

This error was found with a tool in development used to generate
automated pretty-printing functions for ioctl decoding in strace.

f51bde6f0d x86, MCE: Retract most UAPI exports
Retract back most macro definitions which went into the
user-visible mce.h header. Even though those bits are mostly
hardware-defined/-architectural, their naming is not. If we export them
to userspace, any kernel unification/renaming/cleanup cannot be done
anymore since those are effectively cast in stone. Besides, if userspace
wants those definitions, they can write their own defines and go crazy.
