Various improvements are made for collecting congestion statistics:
- Pre-existing logic is replaced with use of an hrtimer for increased
reporting accuracy.
- Congestion timestamp information is reorganized into a single struct.
- Common statistic collection logic is refactored into a helper routine.

There is mishandling of SLI-4 CQE status values larger than what is allowed
by the LPFC_IOCB_STATUS_MASK of 4 bits. The LPFC_IOCB_STATUS_MASK is a
leftover SLI-3 construct and serves no purpose in SLI-4 path.
Remove the LPFC_IOCB_STATUS_MASK and clean up general CQE status handling
in SLI-4 completion paths.

A firmware upgrade does not necessitate dumping of phba->dbg_log[] to kmsg
via LOG_TRACE_EVENT. A simple KERN_NOTICE log message should suffice to
notify the user of successful or unsuccessful firmware upgrade. As such,
firmware upgrade log messages are updated to use KERN_NOTICE instead of
LOG_TRACE_EVENT. Additionally, in order to notify the user of reset type
for instantiating newly downloaded firmware, lpfc_log_msg's default
KERN_LEVEL is updated to 5 or KERN_NOTICE.

When NPIV ports are zoned to devices that support both initiator and target
mode, a remote device's initiated PRLI results in unintended final kref
clean up of the device's ndlp structure. This disrupts NPIV ports'
discovery for target devices that support both initiator and target mode.
Modify the NPIV lpfc_drop_node clause such that we allow the ndlp to live
so long as it was in NLP_STE_PLOGI_ISSUE, NLP_STE_REG_LOGIN_ISSUE, or
NLP_STE_PRLI_ISSUE nlp_state. This allows lpfc's issued PRLI completion
routine to determine if the final kref clean up should execute rather than
a remote device's issued PRLI.

Pre-existing device loss recovery logic via the NLP_IN_RECOV_POST_DEV_LOSS
flag only handled Fabric Port Login, Fabric Controller, Management, and
Name Server addresses.
Fabric domain controllers fall under the same category for usage of the
NLP_IN_RECOV_POST_DEV_LOSS flag. Add a default case statement to mark an
ndlp for device loss recovery.

In dev_loss_tmo callback routine, we early return if the ndlp is in a state
of rediscovery. This occurs when a target proactively PLOGIs or PRLIs
after an RSCN before the dev_loss_tmo callback routine is scheduled to run.
Move clear of the NLP_IN_DEV_LOSS flag before the ndlp state check in such
cases.

Due to a target port D_ID swap, it is possible for the
lpfc_register_remote_port() routine to touch post mortem fc_rport memory
when trying to access fc_rport->dd_data.
The D_ID swap causes a simultaneous call to lpfc_unregister_remote_port(),
where fc_remote_port_delete() reclaims fc_rport memory.
Remove the fc_rport->dd_data->pnode NULL assignment because the following
line reassigns ndlp->rport with an fc_rport object from
fc_remote_port_add() anyways. The pnode nullification is superfluous.

It has been determined that the threaded IRQ API accomplishes effectively
the same performance metrics as blk_irq_poll. As blk_irq_poll is mostly
scheduled by the softirqd and handled in softirq context, this is not
entirely desired from a Fibre Channel driver context. A threaded IRQ model
fits cleaner. This patch replaces the blk_irq_poll logic with threaded
IRQ.

A new RCQE status value indicating DMA failure when transferring
asynchronously received data to an RQE is introduced. Such errors are
unexpected and handlers are updated to log KERN_ERR and dump lpfc's debug
trace buffer to kmsg.

The SCSI version of the abort handler routine, lpfc_abort_handler(), takes
the lpfc_cmd->buf_lock and then phba->hbalock.
Make the same change for the NVMe abort path, lpfc_nvme_fcp_abort(), to
have consistent lock ordering logic between the two abort paths.

Smatch detected a double free path because lpfc_nlp_not_used() releases an
ndlp object before reaching lpfc_nlp_put() at the end of
lpfc_cmpl_els_logo_acc().
Remove the outdated lpfc_nlp_not_used() routine. In
lpfc_mbx_cmpl_ns_reg_login(), replace the call with lpfc_nlp_put(). In
lpfc_cmpl_els_logo_acc(), replace the call with lpfc_unreg_rpi() and keep
the lpfc_nlp_put() at the end of the routine. If ndlp's rpi was
registered, then lpfc_unreg_rpi()'s completion routine performs the final
ndlp clean up after lpfc_nlp_put() is called from lpfc_cmpl_els_logo_acc().
Otherwise if ndlp has no rpi registered, the lpfc_nlp_put() at the end of
lpfc_cmpl_els_logo_acc() is the final ndlp clean up.

In lpfc_sli4_pci_mem_unset(), case LPFC_SLI_INTF_IF_TYPE_1 does not have a

When if_type equals zero and pci_resource_start(pdev, PCI_64BIT_BAR4)
returns false, drbl_regs_memmap_p is not remapped. This passes a NULL
pointer to iounmap(), which can trigger a WARN() on certain arches.
When if_type equals six and pci_resource_start(pdev, PCI_64BIT_BAR4)
returns true, drbl_regs_memmap_p may has been remapped and
ctrl_regs_memmap_p is not remapped. This is a resource leak and passes a
NULL pointer to iounmap().
To fix these issues, we need to add null checks before iounmap(), and
change some goto labels.

which appeared in v6.0, the PCI core has enabled PCIe error reporting for
all devices during enumeration.

pci_enable_pcie_error_reporting() enables the device to send ERR_*
Messages. Since commit f26e58bf6f ("PCI/AER: Enable error reporting when
AER is native"), the PCI core does this for all devices during enumeration,
so the driver doesn't need to do it itself.
Remove the redundant pci_enable_pcie_error_reporting() call from the
driver. Also remove the corresponding pci_disable_pcie_error_reporting()
from the driver .remove() path.
Note that this only controls ERR_* Messages from the device. An ERR_*
Message may cause the Root Port to generate an interrupt, depending on the
AER Root Error Command register managed by the AER service driver.

During tolerance tests that force an HBA to become unresponsive, rmmod
hangs resulting in the inability to remove the driver.
The lpfc_pci_remove_one_s4() routine attempts to submit a clean up mailbox
command via the lpfc_sli4_post_sync_mbox() routine, but ends up waiting
forever for a mailbox register to set its ready bit. Because the HBA is in
an unrecoverable and unresponsive state, the ready bit will never be set.
Create a new routine called lpfc_sli4_unrecoverable_port(), which checks a
port status register's error notification bits.
Use the lpfc_sli4_unrecoverable_port() routine in ready bit check routines
to early return error if port is deemed unrecoverable.
Also, when the lpfc_handle_eratt_s4() handler detects an unrecoverable
state, call the lpfc_sli4_offline_eratt() routine to kick off flushing
outstanding I/O.

A fabric controller can sometimes send an RDP request right before a link
down event. Because of this outstanding RDP request, the driver does not
remove the last reference count on its ndlp causing a potential leak of RPI
resources when devloss tmo fires.
In lpfc_cmpl_els_rsp(), modify the NPIV clause to always allow the
lpfc_drop_node() routine to execute when not registered with SCSI
transport.
This relaxes the contraint that an NPIV ndlp must be in a specific state in
order to call lpfc_drop node. Logic is revised such that the
lpfc_drop_node() routine is always called to ensure the last ndlp decrement
occurs.

When mapped to a target with multiple virtual ports, a link bounce
sometimes results in unsuccessful rediscovery of all of the target's
virtual ports. This is because a succession of repeat RSCNs for the
virtual target ports leaves ndlps in the REG_LOGIN state with the
NLP_REG_LOGIN_SEND flag set. With NLP_REG_LOGIN_SEND set, during the next
PLOGI, the driver will UNREG_RPI. When UNREG_RPI is processed, the driver
can be in the middle of PRLI_ISSUE or MAPPED state resulting in an illegal
state transition by the discovery engine and stalling.
Fix by calling the discovery state machine with DEVICE_RECOVERY event
during RSCN processing. This will set the NLP_IGNR_REG_CMPL bit and
prevent the old REG_LOGIN state from advancing. Then for the new PLOGI
issue, add the check for the NLP_IGNR_REG_CMPL bit to delay issuing the new
PLOGI until the queued REG_LOGIN and UNREG_LOGIN have been processed.

A target vendor array reboot in P2P topology can sometimes result in
unsuccessful rediscovery.
Rework the lpfc_cmpl_els_logo() routine such that when the LOGO completes
as a failure because of driver abort, the LOGO state is still recorded with
the discovery state machine.
This is a small rework to set LOGO completion without forcing a device
removal state change.

Lockdep enabled kernels report a theoretical deadlock state where the
cmf_timer interrupt occurs while the rx_monitor ring is being destroyed.
During rmmod, the cmf_timer is cancelled prior to the
lpfc_rx_monitor_destroy_ring call. This actually eliminates the need to
take the rx_monitor ring lock in lpfc_rx_monitor_destroy_ring. Thus, just
remove lock/unlock of rx_monitor in lpfc_rx_monitor_destroy_ring.

A static code analysis tool flagged the possibility of buffer overflow when
using copy_from_user() for a debugfs entry.
Currently, it is possible that copy_from_user() copies more bytes than what
would fit in the mybuf char array. Add a min() restriction check between
sizeof(mybuf) - 1 and nbytes passed from the userspace buffer to protect
against buffer overflow.

If the &epd_pool->list is empty when executing
lpfc_get_io_buf_from_expedite_pool() the function would return an invalid
pointer. Even in the case if the list is guaranteed to be populated, the
iterator variable should not be used after the loop to be more robust for
future changes.
Linus proposed to avoid any use of the list iterator variable after the
loop, in the attempt to move the list iterator variable declaration into
the macro to avoid any potential misuse after the loop [1].

If kzalloc() fails in lpfc_sli4_cgn_params_read(), then we rely on
lpfc_read_object()'s routine to NULL check pdata.
Currently, an early return error is thrown from lpfc_read_object() to
protect us from NULL ptr dereference, but the errno code is -ENODEV.
Change the errno code to a more appropriate -ENOMEM.

It turns out that commit 596ff4a09b ("cpumask: re-introduce
constant-sized cpumask optimizations") exposed a number of cases of
drivers not checking the result of "cpumask_next()" and friends
correctly.
The documented correct check for "no more cpus in the cpumask" is to
check for the result being equal or larger than the number of possible
CPU ids, exactly _because_ we've always done those constant-sized
cpumask scans using a widened type before. So the return value of a
cpumask scan should be checked with
if (cpu >= nr_cpu_ids)
...
because the cpumask scan did not necessarily stop exactly *at* that
maximum CPU id.
But a few cases ended up instead using checks like
if (cpu == nr_cpumask_bits)
...
which used that internal "widened" number of bits. And that used to
work pretty much by accident (ok, in this case "by accident" is simply
because it matched the historical internal implementation of the cpumask
scanning, so it was more of a "intentionally using implementation
details rather than an accident").
But the extended constant-sized optimizations then did that internal
implementation differently, and now that code that did things wrong but
matched the old implementation no longer worked at all.
Which then causes subsequent odd problems due to using what ends up
being an invalid CPU ID.
Most of these cases require either unusual hardware or special uses to
hit, but the random.c one triggers quite easily.
All you really need is to have a sufficiently small CONFIG_NR_CPUS value
for the bit scanning optimization to be triggered, but not enough CPUs
to then actually fill that widened cpumask. At that point, the cpumask
scanning will return the NR_CPUS constant, which is _not_ the same as
nr_cpumask_bits.
This just does the mindless fix with
sed -i 's/== nr_cpumask_bits/>= nr_cpu_ids/'
to fix the incorrect uses.
The ones in the SCSI lpfc driver in particular could probably be fixed
more cleanly by just removing that repeated pattern entirely, but I am
not emptionally invested enough in that driver to care.

The LLDD and the stack currently process FPINs received from the fabric,
but the stack is not aware of any action taken by the driver to alleviate
congestion. The current interface between the driver and the SCSI stack is
limited to passing the notification mainly for statistics and heuristics.
The reaction to an FPIN could be handled either by the driver or by the
stack (marginal path and failover). Amend the interface to indicate if
action on an FPIN has already been reacted to by the LLDDs or not. Add an
additional flag to fc_host_fpin_rcv() to indicate if the FPIN has been
acknowledged/reacted to by the driver.
Also added a new event code FCH_EVT_LINK_FPIN_ACK to notify to the user
that the event has been acknowledged/reacted by the LLDD driver

Define new FC Link ACQE with new attention types 0x8 (Link Activation
Failure) and 0x9 (Link Reset Protocol Event).
Both attention types are meant to be informational-only type ACQEs with no
action required.
0x8 is reported for diagnostic purposes, while 0x9 is posted during a
normal link up transition when activating BB Credit Recovery feature.
As such, modify lpfc_sli4_async_fc_evt() logic to log the attention types
according to its severity and early return when informational-only
attention types are encountered.

information.

The driver accessed wr_object pointer data, which was initialized into
mailbox payload memory, after the mailbox object was released back to the
mailbox pool.
Fix by moving the mailbox free calls to the end of the routine ensuring
that we don't reference internal mailbox memory after release.

In a large SAN testing configuration, frequent target port toggle tests are
occasionally resulting in missing lun path rediscoveries. An outstanding
PRLI can be inflight when a target RSCN dissappearance occurs, causing the
driver to retry PRLIs using invalid rpi contexts.
Fix by verifying that an ndlp's state was not restarted from PRLI_ISSUE
due to an intermediate RSCN. If not in a valid state, early exit PRLI
completion handling.
The last follow up RSCN indicating target reappearance retriggers
PLOGI/PRLI with a valid rpi context and is expected to succeed in LUN path
rediscovery.

With faulty cables in PT2PT topology, an unintentional ndlp double kref
decrement can occur.
If a FLOGI request is outstanding before the link goes down, the missing
FLOGI_ACC causes an F_Port ndlp to remain in the UNUSED state. During link
down, lpfc_cleanup_rpis() is called and decrements an ndlp kref.
Additionally, when the driver later decides to abort the FLOGI, the FLOGI
completion handler decrements the ndlp kref a second time.
Remove duplicate clean up logic in lpfc_cleanup_rpis() because the updated
FLOGI completion handler already handles the ndlp kref decrement.

The disable_vport() path calls the discovery state machine on all ndlps,
puts them into NPR state, and then calls lpfc_cleanup_rpis() with the
remove flag set. This unintentionally decrements an ndlp's kref twice and
can result in premature release of an ndlp because
lpfc_dev_loss_tmo_handler() triggers clean up of the ndlp again later.
Remove redundant code in disable_vport() that sets all the ndlps to NPR,
and change the call to lpfc_cleanup_rpis() to not remove the ndlps.
lpfc_dev_loss_tmo_handler() will handle final removal of the ndlps.

The local variables called curr_data are incremented, but not actually used
for anything so they are removed.
The return value of lpfc_sli4_poll_eq is not used anywhere and is not
called outside of lpfc_sli.c. Thus, its declaration is removed from
lpfc_crtn.h Also, lpfc_sli4_poll_eq's path argument is not used in the
routine so it is removed along with corresponding macros.

The kernel test robot pointed out non-NULL terminated string possibilities
when using strncpy() in lpfc_xcvr_data_show() routine. Although we
manually set the NULL character after strncpy(), strncpy() usage is
outdated.
Replace all strncpy() usages with the preferred strscpy() API.

When a FLOGI completes with a sequence timeout error, a freed kref ptr
dereference crash can occur due to a timing race involving ndlp referencing
in lpfc_dev_loss_tmo_callbk.
Fix by ensuring the driver accounts for an outstanding FLOGI when dev_loss
is active. Also, don't remove the HBA_FLOGI_OUTSTANDING flag when the
FLOGI is retried to allow the driver to handle the reference counts
correctly in lpfc_dev_loss_tmo_handler.

The dynamic mi_ver value holds the currently configured MI setting. mi_ver
was being displayed as part of the cmf_info sysfs attribute, when the
output string meant to display MI capabilities instead.
Add a mi_cap member in the lpfc_pc_sli4_params structure that will store MI
capabilities during initialization so that cmf_info prints out capabilities
instead of current configuration.

The lpfc_cmf_timer adjusts phba->cmf_link_byte_count periodically and can
artifically inflate bandwidth percent.
During bandwidth calculation, correct for this by setting a cap of logging
a maximum of 100%.
Bandwidth calculation is only used for display under LOG_CGN_MGMT so there
is no expectation of impacts on performance.

Adapter configurations with limited EQ resources may fail to initialize.
Firmware resources are queried in lpfc_sli4_read_config(). The driver
parameters cfg_irq_chann and cfg_hdw_queue are adjusted from defaults if
constrained by firmware resources.
The minimum resource check includes a special allocation for queues such as
ELS, MBOX, NVME LS. However the additional reservation was also incorrectly
applied to EQ resources.
Reordered WQ|CQ|EQ resource checks to apply the special allocation
adjustment to WQ and CQ resources only.

The DUMP_MEMORY mailbox command is implemented for page A0 and A2 to
retrieve transceiver information from firmware.
The mailbox command output is then formatted to print raw data values for
userspace to parse via sysfs.

Commit 5e633302ac ("scsi: lpfc: vmid: Add support for VMID in mailbox
command") introduced allocations for the VMID resources in
lpfc_create_port() after the call to scsi_host_alloc(). Upon failure on the
VMID allocations, the new code would branch to the 'out' label, which
returns NULL without unwinding anything, thus skipping the call to
scsi_host_put().
Fix the problem by creating a separate label 'out_free_vmid' to unwind the
VMID resources and make the 'out_put_shost' label call only
scsi_host_put(), as was done before the introduction of allocations for
VMID.

Updates to the usual drivers (qla2xxx, lpfc, ufs, hisi_sas, mpi3mr,
mpt3sas, target); the biggest change (from my biased viewpoint) being
that the mpi3mr now attached to the SAS transport class, making it the
first fusion type device to do so. Beyond the usual bug fixing and
security class reworks, there aren't a huge number of core changes.

Removed the lpfc_fdmi_attr_entry and lpfc_fdmi_attr_def structures that had
a union causing unintentional zero padding, which required the usage of
__packed. They are replaced with explicit lpfc_fdmi_attr_u32,
lpfc_fdmi_attr_wwn, lpfc_fdmi_attr_fc4types, and lpfc_fdmi_attr_string
structure defines instead of living in a union. This rids of ambiguous
compiler zero padding, and entailed cleaning up bitwise endian
declarations.
As such, all FDMI attribute registration routines are replaced with generic
void *arg and handlers for each of the newly defined attribute structure
types.

On a PCI hotplug capable system, it is possible for scsi_device_put() to
happen after lpfc_pci_remove_one() is called. As a result, the
sdev->host->hostt->module dereference is for a previously freed memory
location because the phba structure containing the hostt template was
already freed when lpfc_pci_remove_one() returned.
Since the lpfc module is still loaded during power slot disable, all
scsi_host_templates should be declared as part of the global data segment
instead of inside the heap allocated phba structure. This way the
sdev->host->hostt memory area is always valid as long as the module is
loaded regardless if PCI hotplug dynamically allocates or frees phba
structures.
Move all scsi_host_templates in the phba structure to global variables.
Create a small helper routine to determine appropriate sg_tablesize during
shost allocation.

When a target makes the mistake of registering a FC4 type with the fabric,
but then rejects a PRLI of that type, the lpfc driver incorrectly retries
the PRLI causing multiple registrations with the transport. The driver
needs to detect the reject reason data and stop any retry.
Rework the PRLI reject scenarios.

Sometimes VMID targets are not getting rediscovered after a port reset.
The iocb is not freed in lpfc_cmpl_ct_cmd_vmid(), which is the completion
function for the appid CT commands. So after a port reset, the count of
sges is less than the expected count of 250. This causes post reset
operation logic to fail and keep the port offline.
Fix by freeing the iocb and kref put for the lpfc_cmpl_ct_cmd_vmid() early
return cases.

In a situation where the node state changes while a REG_LOGIN is in
progress, the LPFC_MBOXQ_t structure is cleared and reused for an
UNREG_LOGIN command to release RPI resources without first freeing the mbuf
pool resource allocated for REG_LOGIN.
Release mbuf pool resource prior to repurposing of the mailbox command
structure from REG_LOGIN to UNREG_LOGIN.

When a FLOGI is received before we have issued our FLOGI, the ACC response
to the received FLOGI is issued with SID 2 instead of the expected fabric
controller SID. Certain target vendors ignore the malformed ACC with SID 2
and wait for a properly filled ACC with a fabric controller SID.
The lpfc_sli_prep_wqe() routine depends on the FC_PT2PT flag to fill in the
fabric controller SID when in PT2PT mode, but due to a previous commit the
flag was getting cleared. Fix by adding a check for the defer_flogi_acc
flag to know whether or not to clear the FC_PT2PT flag on link up.

The if statment check (prli_fc4_req & PRLI_NVME_TYPE) evaluates to true
when receiving a PRLI request for bogus FC4 type codes that happen to have
the 3rd or 5th bit set because PRLI_NVME_TYPE is 0x28. This leads to
sending a PRLI_NVME_ACC even for bogus FC4 type codes.
Change the bitwise & check to an exact == type code check to ensure we send
PRLI_NVME_ACC only for NVME type coded PRLI requests.

When the driver hits an internal error condition returning DID_REQUEUE the
I/O will be retried on the same ITL nexus. This will inhibit multipathing,
resulting in endless retries even if the error could have been resolved by
using a different ITL nexus. Return DID_TRANSPORT_DISRUPTED to allow for
multipath to engage and route I/O to another ITL nexus.

The kernel test robot reported the following sparse warning:

On arm64, atomic_xchg only works on 8-bit byte fields. Thus, the macro
usage of LPFC_RXMONITOR_TABLE_IN_USE can be unintentionally truncated
leading to all logic involving the LPFC_RXMONITOR_TABLE_IN_USE macro to not
work properly.
Replace the Rx Table atomic_t indexing logic with a new
lpfc_rx_info_monitor structure that holds a circular ring buffer. For
locking semantics, a spinlock_t is used.

During a stress offline/online test in PT2PT topology, target rediscovery
can fail with a specific target vendor array.
When the HBA transitions to online mode it is possible to receive an
unsolicited FLOGI before processing the Link Up event. The received FLOGI
will set the defer_flogi_acc_flag, which instructs the driver to wait until
it transmits its own FLOGI before ACKing the received FLOGI. In this
failure scenario, the link up processing clears the set
defer_flogi_acc_flag before we have sent out the FLOGI. As the target has
the higher WWPN and is responsible for sending the PLOGI, the target is
stuck waiting for its FLOGI_ACC that the driver will never send.
Remove the clear of defer_flogi_acc_flag from Link Up event processing. In
this stress test case, the defer_flogi_acc_flag is cleared during the Link
Down event processing anyways.

lpfc_nvmet_prep_abort_wqe() has a lot of common code with
lpfc_sli_prep_abort_xri().
Delete lpfc_nvmet_prep_abort_wqe() as the wqe can be filled out using the
generic lpfc_sli_prep_abort_xri routine(). Add the wqec option to
lpfc_sli_prep_abort_xri() for lpfc_nvmet_prep_abort_wqe().

The RSCN_MEMENTO logic was to workaround a target that does not register
both FCP and NVMe FC4 types at the same time. This caused the
configuration to not produce a second RSCN for the NVMe FC4 type
registration in a timely manner. The intention of the RSCN_MEMENTO flag
was to always signal to try NVMe PRLI.
However, there are other FCP-only target arrays in correctly behaved
configurations that reject the NVMe PRLI followed by a LOGO leading to
never rediscovering the target after an issue_lip (as LOGO causes a repeat
of PLOGI/PRLIs).
Revert the RSCN_MEMENTO patch as it is causing correctly behaved configs to
fail while it exists only to succeed on a misbehaved config.

During a target link bounce test, the driver sees a mismatch between the
NPortId and the WWPN on the node structures (ndlps) involved. When this
occurs, the driver "swaps" the ndlp and new_ndlp node parameters to restore
WWPN/DID uniqueness in the fc_nodes list per vport. However, the driver
neglected to swap the nlp_fc4_type in the ndlp passed to
lpfc_plogi_confirm_nport causing a failure to recover the NVMe PLOGI/PRLI
and ultimately the NVMe paths.
Correct confirm_nport to preserve the fc4 types from the new-ndlp when the
data is moved over ot the ndlp structure.

Disabling FA-PWWN should be effective after port reset, but in some cases
it was found to be impossible to clear FA-PWWN usage without a driver
reload.
Clean up FA-PWWN flag management to make enable and disable of the feature
more robust.

After issuing a LIP, a specific target vendor does not ACC the FLOGI that
lpfc sends. However, it does send its own FLOGI that lpfc ACCs. The
target then establishes the port IDs by sending a PLOGI. lpfc PLOGI_ACCs
and starts the RPI registration for DID 0x000001. The target then sends a
LOGO to the fabric DID. lpfc is currently treating the LOGO from the
fabric DID as a link down and cleans up all the ndlps. The ndlp for DID
0x000001 is put back into NPR and discovery stops, leaving the port in
stuck in bypassed mode.
Change lpfc behavior such that if a LOGO is received for the fabric DID in
PT2PT topology skip the lpfc_linkdown_port() routine and just move the
fabric DID back to NPR.

A use-after-free crash can occur after an ELS LOGO is aborted.
Specifically, a nodelist structure is freed and then
ndlp->vport->cfg_log_verbose is dereferenced in lpfc_nlp_get() when the
discovery state machine is mistakenly called a second time with
NLP_EVT_DEVICE_RM argument.
Rework lpfc_cmpl_els_logo() to prevent the duplicate calls to release a
nodelist structure.

Refactoring and consolidation of abort paths:
- lpfc_sli4_abort_fcp_cmpl() and lpfc_sli_abort_fcp_cmpl() are combined
into a single generic lpfc_sli_abort_fcp_cmpl() routine. Thus, remove
extraneous lpfc_sli4_abort_fcp_cmpl() prototype declaration.
- lpfc_nvme_abort_fcreq_cmpl() abort completion routine is called with a
mismatched argument type. This may result in misleading log message
content. Update to the correct argument type of lpfc_iocbq instead of
lpfc_wcqe_complete. The lpfc_wcqe_complete should be derived from the
lpfc_iocbq structure.

A previous commit assumed all XMIT_SEQ64_WQEs are prepped with the correct
BDE type in word 0-2. However, lpfc_ct_reject_event() routine was missed
and is still filling out the incorrect BDE type.
Fix lpfc_ct_reject_event() routine so that type BUFF_TYPE_BDE_64 is set
instead of BUFF_TYPE_BLP_64.

NVMe I/O problems may be seen on IOMMU enabled platforms. Adapter I/Os
failing with transfer length mismatches.
The sg list processing routine for NVMe I/O is accessing the sg entry
directly for the length and address fields. On some IOMMU platforms,
contigous mappings are compressed to the first sg entry with the sum of the
lengths set to the sg entry dma_length field. The length fields are left
for later use by the unmap call. As such, the driver didn't see the actual
dma_length value, just the first entries length value. Drivers are to use
the sg_dma_length() and sg_dma_address() macros to reference the sg
entry. The macros select the proper length field (dma_length or length) to
reference.
Fix the offending code to use the sg_dma_xxx macros.

When configuring CMF management based on signals instead of FPINs, FPIN
alarm and warning statistics are not tracked.
Change the behavior so that FPIN alarms and warnings are always tracked
regardless of the configured mode.
Similar changes are made in the CMF signal stat accounting logic. Upon
receipt of a signal, only track signaled alarms and warnings. FPIN stats
should not be incremented upon receipt of a signal.

After a link up, it's possible for the switch to change FDMI support (e.g.
FDMI1 vs FDMI2 vs SmartSAN). If the switch reverts to FDMI1, then the
revert is currently not detected.
Additionally, when NPIV is configured, it's possible the physical port's
RHBA is unprocessed by the switch before reciept of an NPIV port issued
RPRT. This causes some switches vendors to reject the NPIV's RPRT.
Fix by reinitializing base FDMI mode on link up, and defer FDMI vport RPRT
submission until after confirming physical port's RHBA is completed.

Currently, VMID registration is configured via module parameters. This
could lead to VMID compatibility issues if two ports are connected to
different brands of switches, as the two brands implement VMID differently.
Make logical changes so that VMID registration is based on common service
parameters from FLOGI_ACC with fabric rather than module parameters.

During large NPIV port testing, it was sometimes seen that not all vports
would log back in to the target device.
There are instances when the fabric is slow to respond to a spam of GID_PT
requests and as a result the SLI PORT may abort the GID_PT request because
the fabric takes so long. lpfc_cmpl_ct_cmd_gid_pt() would enter the
lpfc_err_lost_link() logic and attempt to lpfc_els_flush_rscn(), which is
fine, but forgets to decrement the gidft_inp counter. This results in a
vport->gidft_inp never reaching 0 and never restarting discovery again.
Decrement vport->gidft_inp if lpfc_err_lost_link() is true for both
lpfc_cmpl_ct_cmd_gid_pt() and lpfc_cmpl_ct_cmd_gid_ft().
Increase logging info during RSCN timeout and lpfc_err_lost_link() events.

In GID_PT mode with lpfc_ns_query=1, a race condition between iterating the
vport->fc_nodes list in lpfc_rscn_recovery_check() and cleanup of an ndlp
can trigger a crash while processing the RSCN of another initiator from the
same zone.
During iteration of the vport->fc_nodes list, an ndlp is cleaned up and
released. lpfc_dequeue_node() is called from lpfc_cleanup_node() leading to
a bad ndlp dereference in lpfc_rscn_recovery_check().
Change list_for_each_entry() to list_for_each_entry_safe() in
lpfc_rscn_recovery_check() to protect against removal of an initiator ndlp,
while walking the vport->fc_nodes list.

Upon driver receipt of a CT cmd for type = 0xFA (Management Server) and
subtype = 0x11 (Fabric Device Management Interface), the driver is
responding with garbage CT cmd data when it should send a properly formed
RJT.
The __lpfc_prep_xmit_seq64_s4() routine was using the wrong buffer for the
reject.
Fix by converting the routine to use the buffer specified in the bde within
the wqe rather than the ill-set bmp element.

After running a short external loopback test, when the external loopback is
removed and a normal cable inserted that is directly connected to a target
device, the system oops in the llpfc_set_rrq_active() routine.
When the loopback was inserted an FLOGI was transmit. As we're looped back,
we receive the FLOGI request. The FLOGI is ABTS'd as we recognize the same
wppn thus understand it's a loopback. However, as the ABTS sends address
information the port is not set to (fffffe), the ABTS is dropped on the
wire. A short 1 frame loopback test is run and completes before the ABTS
times out. The looback is unplugged and the new cable plugged in, and the
an FLOGI to the new device occurs and completes. Due to a mixup in ref
counting the completion of the new FLOGI releases the fabric ndlp. Then the
original ABTS completes and references the released ndlp generating the
oops.
Correct by no-op'ing the ABTS when in loopback mode (it will be dropped
anyway). Added a flag to track the mode to recognize when it should be
no-op'd.

During testing with repeated asynchronous resets of the target, an issue
was found when the driver issues a LOGO to disconnect its login and recover
all exchanges. The LOGO command takes a node reference but neglects to
remove it, keeping the node reference count artifically high.
Add a call to lpfc_nlp_put() to lpfc_nlp_logo_unreg() and move the mempool
free call to the routine exit along with the needed put. This is always
safe as this will not be the last reference removed as lpfc_unreg_rpi()
ensures there is an additional reference on the ndlp.

Garbage FCoE CT frames are transmitted on the wire because of bad DMA ptr
addresses filled in the GEN_REQ_WQE.
The __lpfc_sli_prep_gen_req_s4() routine is using the wrong buffer for the
payload address. Change the DMA buffer assignment from the bmp buffer to
the bpl buffer.

The refactoring code converted context information from SLI-3 to SLI-4.
The conversion for the SLI-4 bit field tried to use the old (hacky) SLI3
high/low bit settings. Needless to say, it was incorrect.
Explicitly set the context field to type FCFI and set it in the wqe.
SLI-4 is now a proper bit field so no need for the shifting/anding.

lpfc_sli_issue_iocb(). This call should not have been added as prep_wqe is
called within the issue_iocb routine. So it's called twice now.

If no handler is found in lpfc_complete_unsol_iocb() to match the rctl of a
received frame, the frame is dropped and resources are leaked.
Fix by returning resources when discarding an unhandled frame type. Update
lpfc_fc_frame_check() handling of NOP basic link service.
