b737a22170 most: remove usage of the deprecated ida_simple_xx() API
ida_alloc() and ida_free() should be preferred to the deprecated
ida_simple_get() and ida_simple_remove().

This is less verbose.

1aaba11da9 driver core: class: remove module * from class_create()
The module pointer in class_create() never actually did anything, and it
shouldn't have been requred to be set as a parameter even if it did
something. So just remove it and fix up all callers of the function in
the kernel tree at the same time.

ddb1381018 MOST: cdev: rename 'mod_init' & 'mod_exit' functions to be module-specific
Rename module_init & module_exit functions that are named
"mod_init" and "mod_exit" so that they are unique in both the
System.map file and in initcall_debug output instead of showing
up as almost anonymous "mod_init".

This is helpful for debugging and in determining how long certain
module_init calls take to execute.

5751564085 drivers: most: use LIST_HEAD() for list_head
There's no need to declare a list and then init it manually,
just use the LIST_HEAD() macro.
