9452b1a3ed compat_ioctl: block: handle cdrom compat ioctl in non-cdrom drivers
Various block drivers implement the CDROMMULTISESSION,
CDROM_GET_CAPABILITY, and CDROMEJECT ioctl commands, relying on the
block layer to handle compat_ioctl mode for them.

Move this into the drivers directly as a preparation for simplifying
the block layer later.

When only integer arguments or no arguments are passed, the
same handler can be used for .ioctl and .compat_ioctl, and
when only pointer arguments are passed, the newly added
blkdev_compat_ptr_ioctl can be used.

43bfeb4510 block: sunvdc: don't run hw queue synchronously from irq context
vdc_blk_queue_start() may be called from irq context, so we can't run
queue via blk_mq_start_hw_queues() since we never allow to run queue
from irq context. Use blk_mq_start_stopped_hw_queues(q, true) to fix
this issue.

ff5d1a4209 sunvdc: Remove VLA usage
In the quest to remove all stack VLA usage from the kernel[1], this moves
the math for cookies calculation into macros and allocates a fixed size
array for the maximum number of cookies and adds a runtime sanity check.
(Note that the size was always fixed, but just hidden from the compiler.)

fef912bf86 block: genhd: add 'groups' argument to device_add_disk
Update device_add_disk() to take an 'groups' argument so that
individual drivers can register a device with additional sysfs

e99e88a9d2 treewide: setup_timer() -> timer_setup()
This converts all remaining cases of the old setup_timer() API into using
timer_setup(), where the callback argument is the structure already
holding the struct timer_list. These should have no behavioral changes,
since they just change which pointer is passed into the callback with
the same available pointers after conversion. It handles the following
examples, in addition to some other variations.

Casting from unsigned long:

    void my_callback(unsigned long data)
    {
        struct something *ptr = (struct something *)data;
    ...
    }
    ...
    setup_timer(&ptr->my_timer, my_callback, ptr);

and forced object casts:

    void my_callback(struct something *ptr)
    {
    ...
    }
    ...
    setup_timer(&ptr->my_timer, my_callback, (unsigned long)ptr);

become:

    void my_callback(struct timer_list *t)
    {
        struct something *ptr = from_timer(ptr, t, my_timer);
    ...
    }
    ...
    timer_setup(&ptr->my_timer, my_callback, 0);

Direct function assignments:

    void my_callback(unsigned long data)
    {
        struct something *ptr = (struct something *)data;
    ...
    }
    ...
    ptr->my_timer.function = my_callback;

have a temporary cast added, along with converting the args:

    void my_callback(struct timer_list *t)
    {
        struct something *ptr = from_timer(ptr, t, my_timer);
    ...
    }
    ...
    ptr->my_timer.function = (TIMER_FUNC_TYPE)my_callback;

And finally, callbacks without a data assignment:

    void my_callback(unsigned long data)
    {
    ...
    }
    ...
    setup_timer(&ptr->my_timer, my_callback, 0);

have their argument renamed to verify they're unused during conversion:

    void my_callback(struct timer_list *unused)
    {
    ...
    }
    ...
    timer_setup(&ptr->my_timer, my_callback, 0);

The conversion is done with the following Coccinelle script:

spatch --very-quiet --all-includes --include-headers \
	-I ./arch/x86/include -I ./arch/x86/include/generated \
	-I ./include -I ./arch/x86/include/uapi \
	-I ./arch/x86/include/generated/uapi -I ./include/uapi \
	-I ./include/generated/uapi --include ./include/linux/kconfig.h \

3ee70591d6 sunvdc: prevent sunvdc panic when mpgroup disk added to guest domain
Using mpgroup to define multiple paths for a virtual disk causes multiple
virtual-device-port ports to be created for that virtual device.
Each virtual-device-port port then gets a vdisk created for it by the Linux
sunvdc driver. As mpgroup is not supported by the Linux sunvdc driver it
cannot handle multiple ports for a single vdisk, leading to a kernel panic
at startup.

This fix prevents more than one vdisk per virtual-device-port being created
until full virtual disk multipathing (mpgroup) support is implemented.

2a842acab1 block: introduce new block status code type
Currently we use nornal Linux errno values in the block layer, and while
we accept any error a few have overloaded magic meanings. This patch
instead introduces a new blk_status_t value that holds block layer specific
status codes and explicitly explains their meaning. Helpers to convert from
and to the previous special meanings are provided for now, but I suspect
we want to get rid of them in the long run - those drivers that have a
errno input (e.g. networking) usually get errnos that don't know about
the special block layer overloads, and similarly returning them to userspace
will usually return somethings that strictly speaking isn't correct
for file system operations, but that's left as an exercise for later.

For now the set of errors is a very limited set that closely corresponds
to the previous overloaded errno values, but there is some low hanging
fruite to improve it.

blk_status_t (ab)uses the sparse __bitwise annotations to allow for sparse
typechecking, so that we can easily catch places passing the wrong values.

d0aedcd4f1 vio: fix reuse of vio_dring slot
vio_dring_avail() will allow use of every dring entry, but when the last
entry is allocated then dr->prod == dr->cons which is indistinguishable from
the ring empty condition. This causes the next allocation to reuse an entry.
When this happens in sunvdc, the server side vds driver begins nack'ing the
messages and ends up resetting the ldc channel. This problem does not effect
sunvnet since it checks for < 2.

The fix here is to just never allocate the very last dring slot so that full
and empty are not the same condition. The request start path was changed to
check for the ring being full a bit earlier, and to stop the blk_queue if
there is no space left. The blk_queue will be restarted once the ring is
only half full again. The number of ring entries was increased to 512 which
matches the sunvnet and Solaris vdc drivers, and greatly reduces the
frequency of hitting the ring full condition and the associated blk_queue
stop/starting. The checks in sunvent were adjusted to account for
vio_dring_avail() returning 1 less.

9bce21828d sunvdc: add cdrom and v1.1 protocol support
Interpret the media type from v1.1 protocol to support CDROM/DVD.

For v1.0 protocol, a disk's size continues to be calculated from the
geometry returned by the vdisk server. The geometry returned by the server
can be less than the actual number of sectors available in the backing
image/device due to the rounding in the division used to compute the
geometry in the vdisk server.

In v1.1 protocol a disk's actual size in sectors is returned during the

will always be larger than the former geometry computed size, disks created
under v1.0 will be forwards compatible to v1.1, but not vice versa.

9934c8c045 block: implement and enforce request peek/start/fetch
Till now block layer allowed two separate modes of request execution.
A request is always acquired from the request queue via
elv_next_request(). After that, drivers are free to either dequeue it
or process it without dequeueing. Dequeue allows elv_next_request()
to return the next request so that multiple requests can be in flight.

Executing requests without dequeueing has its merits mostly in
allowing drivers for simpler devices which can't do sg to deal with
segments only without considering request boundary. However, the
benefit this brings is dubious and declining while the cost of the API
ambiguity is increasing. Segment based drivers are usually for very
old or limited devices and as converting to dequeueing model isn't
difficult, it doesn't justify the API overhead it puts on block layer
and its more modern users.

Previous patches converted all block low level drivers to dequeueing
model. This patch completes the API transition by...

* renaming elv_next_request() to blk_peek_request()

* renaming blkdev_dequeue_request() to blk_start_request()

* adding blk_fetch_request() which is combination of peek and start

* disallowing completion of queued (not started) requests

* applying new API to all LLDs

Renamings are for consistency and to break out of tree code so that
it's apparent that out of tree drivers need updating.

43fdf27470 [SPARC64]: Abstract out mdesc accesses for better MD update handling.
Since we have to be able to handle MD updates, having an in-tree
set of data structures representing the MD objects actually makes
things more painful.

The MD itself is easy to parse, and we can implement the existing
interfaces using direct parsing of the MD binary image.

The MD is now reference counted, so accesses have to now take the
form:

	handle = mdesc_grab();

	... operations on MD ...

	mdesc_release(handle);

The only remaining issue are cases where code holds on to references
to MD property values. mdesc_get_property() returns a direct pointer
to the property value, most cases just pull in the information they
need and discard the pointer, but there are few that use the pointer
directly over a long lifetime. Those will be fixed up in a subsequent
changeset.

A preliminary handler for MD update events from domain services is
there, it is rudimentry but it works and handles all of the reference
counting. It does not check the generation number of the MDs,
and it does not generate a "add/delete" list for notification to
interesting parties about MD changes but that will be forthcoming.
