48c3375c5f USB: XHCI: fix memory leak of URB-private data
This patch (as1640) fixes a memory leak in xhci-hcd.  The urb_priv
data structure isn't always deallocated in the handle_tx_event()
routine for non-control transfers.  The patch adds a kfree() call so
that all paths end up freeing the memory properly.

This patch should be backported to kernels as old as 2.6.36, that
contain the commit 8e51adccd4 "USB: xHCI:
Introduce urb_priv structure"

f18f8ed2a9 xhci: Fix TD size for isochronous URBs.
To calculate the TD size for a particular TRB in an isoc TD, we need
know the endpoint's max packet size.  Isochronous endpoints also encode
the number of additional service opportunities in their wMaxPacketSize
field.  The TD size calculation did not mask off those bits before using
the field.  This resulted in incorrect TD size information for
isochronous TRBs when an URB frame buffer crossed a 64KB boundary.

For example:
 - an isoc endpoint has 2 additional service opportunites and
   a max packet size of 1020 bytes
 - a frame transfer buffer contains 3060 bytes
 - one frame buffer crosses a 64KB boundary, and must be split into
   one 1276 byte TRB, and one 1784 byte TRB.

The TD size is is the number of packets that remain to be transferred
for a TD after processing all the max packet sized packets in the
current TRB and all previous TRBs.

For this TD, the number of packets to be transferred is (3060 / 1020),
or 3.  The first TRB contains 1276 bytes, which means it contains one
full packet, and a 256 byte remainder.  After processing all the max
packet-sized packets in the first TRB, the host will have 2 packets left
to transfer.

The old code would calculate the TD size for the first TRB as:

total packet count = DIV_ROUND_UP (TD length / endpoint wMaxPacketSize)
total packet count - (first TRB length / endpoint wMaxPacketSize)

The math should have been:

total packet count = DIV_ROUND_UP (3060 / 1020) = 3
3 - (1276 / 1020) = 2

Since the old code didn't mask off the additional service interval bits
from the wMaxPacketSize field, the math ended up as

total packet count = DIV_ROUND_UP (3060 / 5116) = 1
1 - (1276 / 5116) = 1

Fix this by masking off the number of additional service opportunities
in the wMaxPacketSize field.

This patch should be backported to stable kernels as old as 3.0, that
contain the commit 4da6e6f247 "xhci 1.0:
Update TD size field format."  It may not apply well to kernels older
than 3.2 because of commit 29cc88979a
"USB: use usb_endpoint_maxp() instead of le16_to_cpu()".

760973d2a7 xhci: Fix isoc TD encoding.
An isochronous TD is comprised of one isochronous TRB chained to zero or
more normal TRBs.  Only the isoc TRB has the TBC and TLBPC fields.  The
normal TRBs must set those fields to zeroes.  The code was setting the
TBC and TLBPC fields for both isoc and normal TRBs.  Fix this.

This should be backported to stable kernels as old as 3.0, that contain
the commit b61d378f2d " xhci 1.0: Set
transfer burst last packet count field."

which was allocated dynamically, is now placed where it belongs: in
the private area at the end of struct ehci_hcd.

9debc1793b USB: EHCI: add a name for the platform-private field
This patch (as1642) adds an ehci->priv field for private use by EHCI
platform drivers.  The space was provided some time ago, but it didn't
have a name.

Until now none of the platform drivers has used this private space,
but that's about to change in the next patch of this series.

0f815a0a70 USB: UHCI: fix IRQ race during initialization
This patch (as1644) fixes a race that occurs during startup in
uhci-hcd.  If the IRQ line is shared with other devices, it's possible
for the handler routine to be called before the data structures are
fully initialized.

The problem is fixed by adding a check to the IRQ handler routine.  If
the initialization hasn't finished yet, the routine will return
immediately.

b810075002 usb: gadget: FunctionFS: Fix missing braces in parse_opts
Add missing braces around an if block in ffs_fs_parse_opts. This broke
parsing the uid/gid mount options and causes mount to fail when using
uid/gid. This has been introduced by commit b9b73f7c (userns: Convert usb
functionfs to use kuid/kgid where appropriate) in 3.7.

c2c9caa947 usb: gadget: fsl_mxc_udc: replace MX35_IO_ADDRESS to ioremap
As mach/hardware.h is deleted, we can't visit platform code at driver.
It has no phy driver to combine with this controller, so it has to use

As mach/hardware.h is deleted, we need to use platform_device_id to
differentiate SoCs. Besides, one cpu_is_mx35 is useless as it has

091a62c9b3 usb: musb: cppi_dma: drop '__init' annotation
This patch fixes the following:

WARNING: vmlinux.o(.text+0x1e709c): Section mismatch in reference from the funct
ion dma_controller_create() to the function .init.text:cppi_controller_start()
The function dma_controller_create() references
the function __init cppi_controller_start().
This is often because dma_controller_create lacks a __init
annotation or the annotation of cppi_controller_start is wrong.

This warning is there due to the deficiency in the commit 07a67bbb (usb: musb:
Make dma_controller_create __devinit).

Since the start() method is only called from musb_init_controller() which is
not annotated, drop '__init' annotation from cppi_controller_start() and also
cppi_pool_init() since it gets called from that function, to avoid another
section mismatch warning...

37f13561de xfs: recalculate leaf entry pointer after compacting a dir2 block
Dave Jones hit this assert when doing a compile on recent git, with
CONFIG_XFS_DEBUG enabled:

XFS: Assertion failed: (char *)dup - (char *)hdr == be16_to_cpu(*xfs_dir2_data_unused_tag_p(dup)), file: fs/xfs/xfs_dir2_data.c, line: 828

Upon further digging, the tag found by xfs_dir2_data_unused_tag_p(dup)
contained "2" and not the proper offset, and I found that this value was
changed after the memmoves under "Use a stale leaf for our new entry."
in xfs_dir2_block_addname(), i.e.

                        memmove(&blp[mid + 1], &blp[mid],
                                (highstale - mid) * sizeof(*blp));

overwrote it.

What has happened is that the previous call to xfs_dir2_block_compact()
has rearranged things; it changes btp->count as well as the
blp array.  So after we make that call, we must recalculate the
proper pointer to the leaf entries by making another call to
xfs_dir2_block_leaf_p().

Dave provided a metadump image which led to a simple reproducer
(create a particular filename in the affected directory) and this
resolves the testcase as well as the bug on his live system.

The int casts here make it easy to trigger an assert with a large
soft limit. For example, set a >4TB soft limit on an empty volume
to reproduce a (0 > -x) comparison due to an overflow of

d44d9bc68e xfs: use b_maps[] for discontiguous buffers
Commits starting at 77c1a08 introduced a multiple segment support
to xfs_buf. xfs_trans_buf_item_match() could not find a multi-segment
buffer in the transaction because it was looking at the single segment
block number rather than the multi-segment b_maps[0].bm.bn. This
results on a recursive buffer lock that can never be satisfied.

This patch:
 1) Changed the remaining b_map accesses to be b_maps[0] accesses.
 2) Renames the single segment b_map structure to __b_map to avoid
    future confusion.

3a55fb0d9f Tell the world we gave up on pushing CC_OPTIMIZE_FOR_SIZE
In commit 281dc5c5ec ("Give up on pushing CC_OPTIMIZE_FOR_SIZE") we
already changed the actual default value, but the help-text still
suggested 'y'. Fix the help text too, for all the same reasons.

Sadly, -Os keeps on generating some very suboptimal code for certain
cases, to the point where any I$ miss upside is swamped by the downside.
The main ones are:

 - using "rep movsb" for memcpy, even on CPU's where that is
   horrendously bad for performance.

 - not honoring branch prediction information, so any I$ footprint you
   win from smaller code, you lose from less code density in the I$.

 - using divide instructions when that is very expensive.

If the default iosched is built as module, the kernel may deadlock
while trying to load the iosched module on device probe if the probing
was running off async.  This is because async_synchronize_full() at
the end of module init ends up waiting for the async job which
initiated the module loading.

 async A				modprobe

 1. finds a device
 2. registers the block device
 3. request_module(default iosched)
					4. modprobe in userland
					5. load and init module
					6. async_synchronize_full()

Async A waits for modprobe to finish in request_module() and modprobe
waits for async A to finish in async_synchronize_full().

Because there's no easy to track dependency once control goes out to
userland, implementing properly nested flushing is difficult.  For
now, make module init perform async_synchronize_full() iff module init
has queued async jobs as suggested by Linus.

This avoids the described deadlock because iosched module doesn't use
async and thus wouldn't invoke async_synchronize_full().  This is
hacky and incomplete.  It will deadlock if async module loading nests;
however, this works around the known problem case and seems to be the
best of bad options.

cbc0dd1 "s390/pci: CHSC PCI support for error and availability events"
introduced a new SEI notification type as part of pci support.
The way SEI was called with nt2 and nt0 consecutive broke the nt0
stuff used for channel subsystem notifications.

The reason why this was broken with the mentioned patch is that you
cannot selectively disable type 0 notifications (so even when asked
for type 2 only, type 0 could be presented).

The way to do it is to tell SEI which types of notification you can
process and -this is the important part- look at the SEI result which
notification type you actually received.

ed4f20943c s390/time: fix sched_clock() overflow
Converting a 64 Bit TOD format value to nanoseconds means that the value
must be divided by 4.096. In order to achieve that we multiply with 125
and divide by 512.
When used within sched_clock() this triggers an overflow after appr.
417 days. Resulting in a sched_clock() return value that is much smaller
than previously and therefore may cause all sort of weird things in
subsystems that rely on a monotonic sched_clock() behaviour.

To fix this implement a tod_to_ns() helper function which converts TOD
values without overflow and call this function from both places that
open coded the conversion: sched_clock() and kvm_s390_handle_wait().

6f54c36132 ALSA: hda/hdmi - Work around "alsactl restore" errors
When "alsactl restore" is performed on HDMI codecs, it tries to
restore the channel map value since the channel map controls are
writable.  But hdmi_chmap_ctl_put() returns -EBADFD when no PCM stream
is assigned yet, and this results in an error message from alsactl.
Although the error is harmless, it's certainly ugly and can be
regarded as a regression.

As a workaround, this patch changes the return code in such a case to
be zero for making others happy.  (A slight excuse is: when the chmap
is changed through the proper alsa-lib API, the PCM status is checked
there anyway, so we don't have to be too strict in the kernel side.)

8aef33a7cf cpuidle: remove the power_specified field in the driver
We realized that the power usage field is never filled and when it
is filled for tegra, the power_specified flag is not set causing all
of these values to be reset when the driver is initialized with
set_power_state().

However, the power_specified flag can be simply removed under the
assumption that the states are always backward sorted, which is the
case with the current code.

This change allows the menu governor select function and the
cpuidle_play_dead() to be simplified.  Moreover, the
set_power_states() function can removed as it does not make sense
any more.

Drop the power_specified flag from struct cpuidle_driver and make
the related changes as described above.

As a consequence, this also fixes the bug where on the dynamic
C-states system, the power fields are not initialized.
