scsi: hpsa: correct ioaccel2 chaining

- set ioaccel2_sg_element member 'chain_indicator' to IOACCEL2_LAST_SG for
  the last s/g element.

- set ioaccel2_sg_element member 'chain_indicator' to IOACCEL2_CHAIN when
  chaining.
